Chronology of Events <br /> 0 5/27/10, :ooam—Catherine Laurent (CL) reported to Rick Pomroy (PA)aria e-mail that a leak <br /> had occurred the previous evening.The e-mail was forwarded to Bufftree immediately and <br /> Bufftree notified their HVAC contractor. <br /> • 27/10, : oam to 10: oarn—ISP reset with Brad Tripp (Mashpee) and Wayne Picard (DDS/HVAC <br /> sub)to begin assessing the situation. <br /> 0 5/27/10, :o prn—We met with project design engineer, Dorn Pun ello (GG D) -to review the <br /> HVAC system. Dora found a number of items with the setup of the HVAC fan unit which <br /> collectively caused the over condensation and leak. One major item was that the unit was not <br /> correctly programmed in the unoccupied mode which meant that dehumidification was not <br /> occurring when the building eras not o % piceu.This item was repr gt a rin�u dui ing-Leh situ <br /> visit.Also, it was agreed during the visit that the system would be programmed run in the <br /> occupied anode 24/7 for Friday and the long holiday weekend in efforts to dehumidify the <br /> building. <br /> 0 5/27/10, :37prn—GGD issued an e-mail message identifying the corrections ne'cessary to the <br /> system. This e-mail was distributed to all parties and a game plan was asked of Bufftree/DDS for <br /> implementation of the GGD points. <br /> • 6/1/10., 9:o am—PA was on site to check on status of the building after the long weekend. <br /> Humidity level was lower, but smell still existed. <br /> 0 6/2/10,2: open—PAI, Bufftree, DDS (HVAC contractor),ABS (automatic temperature control <br /> sub),the HVAC manufacturer met to review the system again. DDS made the corrections to the <br /> condensate trap and ABS made corrections to the controls.The Plumbing sub checked for <br /> passible vent lino leaks which may cause the smell. All plumbing vent lines checked out.The <br /> humidity level was expected to drop even lower and hopefully the smell would begin t <br /> dissipate. From 2prn to approximately :Bprn the some windows and doors were opened to <br /> i <br /> assist in the ventilation and fresh air. <br /> 6/3/10,9:15a m—CL advised me that several complaints had been re eiv d from tine Iibra ry staff <br /> regarding the smell. i explained that the best thing to do at this time was for us to give <br /> i • <br /> unimpeded access to the contractor to bring in whatever equipment necessary to get the <br /> building dried out.The Torn opted to close the building through Sunday June 6 to and reopen <br /> Ih <br /> June 7. . B fftr e was notified immediately. <br /> 6/3/10, :o pm—Bufftree was on site-with World Wide Drying a cleaning/dehumidification <br /> corn pa ny) t establish ars auction plan to correct the smell. Brad Trip was also on site with the <br /> Barnstable County Health Agent to review the condition on site.The health agent indicated that <br /> air quality was not a concern.The humidity levels had dropped considerably and World Wide <br /> felt that the smell would soon dissipate without adding any additional drying equipment to the <br /> building. World Wide mapped out a proposed process with the health agent which fiat attempt <br /> to draw out the smells using charcoal type canisters, then ifunsuccessful to introduce de- . <br /> ionization units to break the carbon molecules down and then if still unsuccessful to introduce <br /> an anti-microbial cleaning agent to the building.The health agent agreed with the approach and <br /> had previous successful'a perience with the de-ionization process.World Wide would return on <br /> 6/4 and install several charcoal type canisters as the fiat step.The hope was that the canisters <br /> would eliminate the smell by Monday morning. <br /> 6/4/10, 10: oam—PA, Bufftree, DDS met with the GGD to review all of the HVAC corrections, <br /> double check the programming changes and review the operation of the HVAC system. GGD <br /> confirmed that the humidity level was within design standards and the system was operating <br /> correctly. GGD did ask that the Town set up remote IT availability for both therm and ABS so that <br />
