3. <br /> ARTICLE II <br /> APPLICATIONS TO THE BOARD <br /> SECTION I: Pr•ocedur•es far• filing Applications for• Special Permits, <br /> Variances or• Appeals under• MGL Chapter 40A. <br /> APPLICATIONS: <br /> 1. Application forms ar•e to be completed in full and two <br /> copies filed and stamped in the office of the Town Cler•k.. <br /> 2. Filing Fee: <br /> Each Application must be accompanied by a filing fee <br /> of $100 per• lot, payable by check to the Town of Mashpee. <br /> 3. Each Application must be accompanied by two sets of plans <br /> prepared and signed by a certified engineer• or• surveyor <br /> showing: <br /> a. the dimensions of the land <br /> b. the exact location of all existing and/or proposed <br /> buildings on the land <br /> C. drainage, utilities including septic plans and paved <br /> surfaces <br /> d. all setback distances of structures from the street, <br /> lot lines, wetlands, water body or• watercourse with <br /> proper identification <br /> e. calculation of percent of lot coverage of present and <br /> proposed structures <br /> f. parking spaces, aisles dimensions, and flow of traffic <br /> noted by arrows should be included on all Commercial <br /> Applications <br /> g. zoning and current uses of the site and adjacent <br /> properties <br /> 4. When the Applicant is not the owner of the property, the <br /> Applicant shall provide the Board with proof of legal <br /> interest in the property, i.e. agent, trustee, leaseholder <br /> or• pending purchase and sale agreement. <br /> 5. It is the responsibility of the Applicant to submit <br /> additional copies of the Application and plans to other <br /> Town Boards as required in the Zoning By-laws. These <br /> plans should be submitted at the same time the Application <br /> is filed with the Town Clerk and it is recommended they be <br /> sent by Certified Mail. <br />
