• 6. <br /> SECTION II . - Applications for Special Permits under .Chapter 40B <br /> Comprehensive Permits <br /> 1 . Application forms are to be completed in full and two <br /> copies filed and stamped in the office of the Town <br /> Clerk . <br /> 2 . Applications must be accompanied by a filing fee of <br /> $100 per unit payable by check to the Town of Mashpee. <br /> 3 . Each Application must be accompanied by 15 sets of <br /> plans prepared and signed by a certified engineer or <br /> surveyor showing : <br /> a. the dimensions of the land <br /> b. the exact location of all existing and/or proposed <br /> buildings on the land <br /> C . drainage, utilities and paved surfaces <br /> d . all setback distances of structures from the <br /> street, lot lines, wetlands, water body or <br /> watercourse with proper identification <br /> e . calculation of percent of lot coverage of present <br /> and proposed structures <br /> f. parking spaces, aisles dimensions, and flow of <br /> traffic noted by arrows should be included on <br /> commercial Applications <br /> g . zoning and current uses of the site and adjacent <br /> properties <br /> 4 . When the Applicant is not the owner of the property the <br /> Applicant shall provide the Board with proof of legal <br /> interest in the property i .e . agent, trustee, <br /> leaseholder or pending purchase and sale agreement . <br /> 5 . Documents sufficient to prove status as non-profit, <br /> limited dividend, or public agency . <br /> 6 . FHA or MHFA site approval or other document showing <br /> agency ' s interest . <br /> 7 . Request for Comprehensive Permit shall include <br /> requested exceptions to local codes, ordinances or <br /> regulations . <br /> 8. Statement of local need for such a development; any <br /> documents to substantiate shall be included . <br />
