The imne•diate <br /> supervisor ( rat,er ) shall rat - the subord,{nate employee and <br /> forward the evaluation to a Re-viewino Officer, who is <br /> normally the Rater ' s supervisor . The Reviewing Officer will <br /> forward the completed evaluari-,iii to the office of rhe chief <br /> of Police who will maintain evaluation , in the employee ' s <br /> personnel file for five years after the employee terminates <br /> employment with the Town of Mashpto. <br /> 2 ) Probationary employeo:> will be evaluated on a <br /> bi -monthly basis until the c, nmplerion of the probationary <br /> term of service at which time they will be r?valuated <br /> semi-annually with the non prnbetion.iry work force . <br /> Immediate supervisors in comm.,ud of probati,inary employees <br /> will consult with the Mashpee Police U-?partment Training <br /> Council and Field Training orris>rs for written training <br /> assessment reports of th- sublect employee in order that <br /> training needs can be assessed and er . usted . Training status <br /> reports may be included with the evaluation form . An <br /> employee may be rated on an interim basis in addition to the <br /> semi -annual or bi-monthly periods for reasons of a varying <br /> nature to include poor or exr_eptional performance, a change <br /> in assignments, or step rate increase? . In all cases, the <br /> rating period will be clearly annotated in the spaces <br /> provided on the Officer Appr.ir ::al Form . <br /> 3 ) All evaluations shall b- annotated ?n the attached <br /> Mashpee Police Department Offr �-er Appraisal e'orm completed <br /> copies of which will remain I:nNFIDENTIAL and unavailable to <br /> personnel not, related to the r•it, ir,ml!evaluation process . A <br /> copy shall be made available for the rated employee .
