This <br /> counseling may occur at any time during the rating period <br /> when deemed necessary : <br /> 2 ) Raters shall be responsibly for providing a <br /> completed copy of the Appraisal Form to the employee and <br /> forwarding the original form to the REVIEWER . <br /> 3 ) Raters shall be prepared to substantiate ratings at <br /> the unsatisfactory level , to advise the employee of <br /> unsatisfactory performance and to d._Luie actions necessary to <br /> improve the performance of the subordinate . Raters should <br /> fully understand the criteria set f'orth .Within the <br /> evaluation/appraisal system and guard against any personality <br /> conflicts, prejudices, or pre-- formed attitudes towards <br /> subordinates . Raters should filly substantiate ratings in <br /> the lowest or highest categories and give specific reasons <br /> through narrative comment in the comment sections provided <br /> within the form . <br /> 4 ) Raters should mak? .p*rt.icular references. to extra <br /> duties , responsibilities, and assignm•:•nts of a subordinate <br /> and the manner in which they a.•complish said tasks . <br /> 5 ) The Performance Appraisa ) Interview should set <br /> direction, discuss organization, set objectives for <br /> promotion, identify training r-quirements, set goals and <br /> standards, and recognize perf••rmance levels . <br /> Reviewing Officers : <br /> l ) Reviewing officers shall be responsible for <br /> screening all evaluation reports received from rating <br /> officers regarding fairness , accuracy, impartiality of - <br /> ratings given, their participation in counseling and guidance <br /> provided to the rated employees . Reviewing officers will <br /> ensure that the rater applies ratings uniformly and that all <br /> pertinent information is completed, that the rated employee <br /> signed the form and that the rated employee was provided a <br /> copy . The reviewing officer shall forward the completed <br /> officer evaluation form to the ofti,e of the Chief of Police. <br /> Rated Employee : <br /> Each evaluated employee will read and ensure <br /> understanding of his/her evaluation completed by his/her <br /> supervisor and the comments of the reviewer . Any questions, <br /> misunderstanding should be addressed firstly to the rating <br /> officer . The signing of the evrtli•ati •n form indicates only <br /> that the rated employee has r-r•ad the report and will not <br /> imply agreement or disagreement with its content . if an <br /> employee refuses to sign the the rating ottrcer will <br /> so note the refusal and nota it,,- or r=•.neon : r"r th'• <br />
