BENEFITS Educational Benef it Program The purpose of the Educational Benefit Program of the Town of Mashpee is to provide an incentive to full-time employees employed for at least one year in order to improve per form- ance effectiveness in their present positions and prepare employees for positions of greater related responsibility. Eligible employees shall be entitled to reimbursements for up to 10 of the cost of tuition and fees per employee per~ term upon satisfactory completion of approved courses of instruction accredited by the New England Association of Colleges and Secondary Schools, To be reimbursed for tuition and fees, eligible employees must first receive approval of their supervisor and Personnel Board and complete the Request fore Educational Approval Form prior to registration or enrollment which can be obtained from the Personnel Department) .. E ployees should forward the receipted bill for tuition and fees as evidence of having completed the course o the appropriate Department Head so that the receipts can be processed to the Personnel Office. "Satisfactory completion of course(s)" shall be defined a the receipt of a certificate Grade "C". For determination of eligibility, please refer to appropriate policy or collective bargaining inin agreement. Group Health In ur^ance Per anent employees who regularly work 20 or more hours per eels, are eligible for Group Wealthy Insurance. The purpose of Group Health Insurance is to provide the employee and his/her family with protection against non--occupational illness or injury. (For example, ho p i tal l zzat ion, medical . and surgical treatment and medical services. ) There is one type of insurance plan: Blue Cross/Blue Shield Master Health Plus. The Town of Mashpee contributes seventy-fire 5%) percent of the premium for group insurance coverage and the employee contributes twenty-five ( 5 ) percent. f an employee has joined the flue dross/flue hie ld lan, his/her coverage will begin upon application on the first of the month following the date of hire with a 240 day waiting period before benefits are provided for most conditions. Certain conditions may qualify for immediate benefits if you receive intial treatment on or after the first day of your membership ship and it is the only condition treated. Examples of T immediate benefit conditions are accidental injuries requiring immediate tr-eatment, acute appendicitis, acute rheumatoid arthritis and most forms of cancer.* *When your begin employment with the Town, you will be given booklet that explains the Health Insurance benefit in detai l I so 6_1 -
