BENEFITS <br /> Educational Benef it Program <br /> The purpose of the Educational Benefit Program of the Town <br /> of Mashpee is to provide an incentive to full-time employees <br /> employed for at least one year in order to improve per form- <br /> ance effectiveness in their present positions and prepare <br /> employees for positions of greater related responsibility. <br /> Eligible employees shall be entitled to reimbursements for up <br /> to 10 of the cost of tuition and fees per employee per~ term <br /> upon satisfactory completion of approved courses of <br /> instruction accredited by the New England Association of <br /> Colleges and Secondary Schools, <br /> To be reimbursed for tuition and fees, eligible employees <br /> must first receive approval of their supervisor and Personnel <br /> Board and complete the Request fore Educational Approval Form <br /> prior to registration or enrollment which can be obtained <br /> from the Personnel Department) .. E ployees should forward the <br /> receipted bill for tuition and fees as evidence of having <br /> completed the course o the appropriate Department Head so <br /> that the receipts can be processed to the Personnel Office. <br /> "Satisfactory completion of course(s)" shall be defined a <br /> the receipt of a certificate Grade "C". <br /> For determination of eligibility, please refer to appropriate <br /> policy or collective bargaining inin agreement. <br /> Group Health In ur^ance <br /> Per anent employees who regularly work 20 or more hours per <br /> eels, are eligible for Group Wealthy Insurance. The purpose <br /> of Group Health Insurance is to provide the employee and <br /> his/her family with protection against non--occupational <br /> illness or injury. (For example, ho p i tal l zzat ion, medical . <br /> and surgical treatment and medical services. ) There is one <br /> type of insurance plan: Blue Cross/Blue Shield Master Health <br /> Plus. The Town of Mashpee contributes seventy-fire 5%) <br /> percent of the premium for group insurance coverage and the <br /> employee contributes twenty-five ( 5 ) percent. <br /> f an employee has joined the flue dross/flue hie ld lan, <br /> his/her coverage will begin upon application on the first of <br /> the month following the date of hire with a 240 day waiting <br /> period before benefits are provided for most conditions. <br /> Certain conditions may qualify for immediate benefits if you <br /> receive intial treatment on or after the first day of your <br /> membership ship and it is the only condition treated.
