Glen Harrington <br /> stated that a request for a temporary food permit has been submitted by Uncle Jay's Twisted Fork, <br /> Inc. The temporary food permit was submitted for use at Naukabout Brewery but the owner has <br /> requested a one-time annual permit fee of$100.00 for three events.The ServSafe certificate and <br /> the allergy awareness certificate were requested but not yet provided. The proposed menu <br /> includes BBQ roasted pig for pork sandwiches. The BBQ grills are reportedly closed, so there is no <br /> open pit-style BBQ. Mr. Harrington's recommended that the board approve the request pending <br /> the submission of the required information and an inspection by a Health Agent. <br /> The applicant, Jay Powell, addressed the board, stating that both the ServSafe and allergy <br /> awareness certificates would be sent via fax the following day. <br /> There being no further comments, Laurel Almquist made a Motion to approve the Request by <br /> Uncle Jay's Twisted Fork, Inc, for a temporary food permit annual fee, provided the required <br /> documents are submitted and an inspection is performed by a Health Agent.
