Town of Mashpee <br /> f <br /> J4 <br /> i <br /> Y } 16 G7 <br /> -eat Neck Road North <br /> LEMaslipee, Massachusetts 02649 <br /> 4 Y <br /> BOARD of HEALTH PUBLIC SESSION <br /> Minutes of the Meeting of September 7, 2017 <br /> Board Members present: Also Present: <br /> Brian Baurngaertel, chair Glen Harrington, Health Agent <br /> Laurel Almquist, co-chair <br /> Mallory Langler, clerk <br /> Chair Brian Baumgaertel called the meeting to order at 7:00 Ili. <br /> APPOINTMENTS <br /> 7:15 PM — Nitrogen Aggregation Loading Pian Request—232 cotuit Road. <br /> Glen Harrington stated that a request for a nitrogen aggregation plan has been submitted for 232 Cotult road. <br /> The facility lard has an area of 9,750 square feet. The credit land from 29 Melissa Avenue has an area of <br /> 0} 50 square feet. Therefore, three bedrooms would be allowed on the total facility land and credit land of <br /> 0,000 square feet. The plans and grant documents have been reviewed and comments were made. The o <br /> review fee has been paid, and the septic design plan will be submitted at a later date. <br /> --Matt costa, Cape and Wands Engineering, representing he applicant of 232,cotuit load, introduced himself <br /> to the Board. <br /> 11,IIr. costa commented that the nitrogen aggregation loading plan, sometimes referred to as facility aggregation <br /> plan is performed for the purposes of meeting nitrogen loading requirements under Title V. These properties <br /> are located in a Zone 11; so, there is a nitrogen loading limitation of one bedroom per 10,000 square feet. Title <br /> has provisions to allover the transfer of credit area from one property to another as long as they are within the <br /> same Zone II in order to achieve the nitrogen aggregation load of one bedroom per 10,000 square feet. <br /> Laurel Almquist made a notion that the Board of Health approve the Nitrogen Aggregation Loading Flan <br /> Request for 232 cotuit Road with the conditions that; <br /> 1. The survey plans be stamped prior to recording at the Registrar of Creeds. <br /> . The Title V design plan for the septic system shall be submitted and approved at a later date. <br /> 3. The Grant of Title V Restriction and Easement documents shall be executed, recorded and submitted <br /> to the Board of Health within 30 days of recording and prior to issuance of a Disposal System <br /> Construction Permit. <br /> Mallory Langler seconded the motion. The motion was unanimously approved. <br /> 7-:20 PM —Appeal the Application of the 4' SAS to GW requirement: 24 Wiisona Grove. <br /> Glen Harrington commented that on July 19, 2017, Frank Nunes performed a septic inspection at 24 Wilsons <br /> Grove where he identified the soil Absorption System having less than the required four feet to groundwater. <br /> Therefore, the system was deemed in failure as a result of the inspection* The realtor, Mark Thompson, <br /> contacted Mr. Harrington to explain the situation and requested further review. Mr. Harrington offered to survey <br /> the elevations of the system and groundwater and determined that the soil Absorption System was 'g" to <br /> groundwater. The benchmark at 36 Wilson's Grove was used since a recent Title V system was installed and <br /> 1 <br />
