Minutes of Meeting of Mashpee Plan Review Committee Held Tuesday, January 2, 2018 Mashpee Town Hall Waquoit Meeting Room Plan Review Members Present: Michael Mendoza , Building Commissioner Tom Fudala, Town Planner Catherine Laurent, DPW Director Glen Harrington, Health Agent John Phelan, Deputy Fire Chief Andrew McManus, Conservation Agent Audience: Mary Ellen and Don MacDonald — Mashpee residents. Michael Mendoza opened the Public Hearing at 9:05 a.m. Project Address: 31 Maple Street Business Name: Fazio Therapy owner/Agent: Jennifer Fazio Representative: Jennifer Fazio Request: Residential Massage Clinic Jennifer Fazio appeared and stated she is an orthopedic therapist and occupational therapist. Mike Mendoza asked her to describe what she intends to be doing at this Maple Street location. She said the house has two levels the first level will be the clinic; the second level is her home. She said she does orthopedic massage and since it is very intense, draining type of work, she will see only about 3 to 5 clients a week and will work the rest of the week outside the home. Jennifer stated she is licensed by the State of Massachusetts. Mike Mendoza noted there are 2 sets of Site Plans one plan in the book and one large Site Plan. Mike said they do not match. Jennifer said the book plan was what she drew to the best of her ability. Catherine Laurent commented that the large Plan is probably the record Plan when the house was built in 1989. It was decided to reference the image that Jennifer Fazio presented. Mike informed the Committee that he had visited the Site about a month or so ago and noted that the door going into the garage has to have a handicap ramp constructed so public access can be granted into that entrance. Also, he told her that outside at the front of the garage a doorbell has to be installed, which has to be at the proper height for handicap access. Emergency lights within the garage and inside the clinic area are required as discussed. Catherine Laurent said her concern would be parking. Two parking spaces are needed for the dwelling. Jennifer Fazio said she only sees one client at a time. Catherine advised that parking is to be on-site; there is to be no on-street parking. Jennifer Fazio said she can get 8 cars on her d riveway.
