Minutes of Meeting of <br /> Mashpee Plan Review Committee <br /> Held Tuesday, January 2, 2018 <br /> Mashpee Town Hall Waquoit Meeting Room <br /> Plan Review Members Present: <br /> Michael Mendoza , Building Commissioner <br /> Tom Fudala, Town Planner <br /> Catherine Laurent, DPW Director <br /> Glen Harrington, Health Agent <br /> John Phelan, Deputy Fire Chief <br /> Andrew McManus, Conservation Agent <br /> Audience: Mary Ellen and Don MacDonald — Mashpee residents. <br /> Michael Mendoza opened the Public Hearing at 9:05 a.m. <br /> Project Address: 31 Maple Street <br /> Business Name: Fazio Therapy <br /> owner/Agent: Jennifer Fazio <br /> Representative: Jennifer Fazio <br /> Request: Residential Massage Clinic <br /> Jennifer Fazio appeared and stated she is an orthopedic therapist and occupational therapist. Mike <br /> Mendoza asked her to describe what she intends to be doing at this Maple Street location. She said <br /> the house has two levels the first level will be the clinic; the second level is her home. She said she <br /> does orthopedic massage and since it is very intense, draining type of work, she will see only about 3 <br /> to 5 clients a week and will work the rest of the week outside the home. Jennifer stated she is <br /> licensed by the State of Massachusetts. <br /> Mike Mendoza noted there are 2 sets of Site Plans one plan in the book and one large Site Plan. <br /> Mike said they do not match. Jennifer said the book plan was what she drew to the best of her ability. <br /> Catherine Laurent commented that the large Plan is probably the record Plan when the house was <br /> built in 1989. It was decided to reference the image that Jennifer Fazio presented. <br /> Mike informed the Committee that he had visited the Site about a month or so ago and noted that the <br /> door going into the garage has to have a handicap ramp constructed so public access can be granted <br /> into that entrance. Also, he told her that outside at the front of the garage a doorbell has to be <br /> installed, which has to be at the proper height for handicap access. Emergency lights within the <br /> garage and inside the clinic area are required as discussed. <br /> Catherine Laurent said her concern would be parking. Two parking spaces are needed for the <br /> dwelling. Jennifer Fazio said she only sees one client at a time. Catherine advised that parking is to <br /> be on-site; there is to be no on-street parking. Jennifer Fazio said she can get 8 cars on her <br /> d riveway. <br />
