There being no questions or revisions, Laurel Almquist moved to approve the minutes of the May <br /> 2, 2019 Public Session Meeting. Motion seconded by Mallory Langler. VOTE: -Unanimous (3-0). <br /> 2. Sign Expense Warrants. There were no expense warrants. <br /> 3. Request for variance from Recreational Camp Regulations, Boys& Girls Club of Cape Cod, Mr, <br /> Harrington commented that the applicant is requesting a variance from the Camp Regulations to <br /> increase their attendance to 140 campers with only four(4) sinks. The board has approved this <br /> variance since 2009 for 130 campers. The number has increased to 20 over the minimum <br /> requirement, No complaints were received at the 130 camper level. Mr. Harrington also <br /> reminded the board that the Boys & Girls Club typically requests a waiver of the camp fee of <br /> $100.00, and recommended this matter be addressed that at the same time. <br /> There being no further comments or discussion, Mallory Langler moved to approve the request <br /> for variance for the Boys&Girls Club of Cape Cod with the following conditions: 1) an inspection <br /> be performed by a Health Agent during operation; and 2) any complaints regarding convenience <br /> of using the bathroom facilities shall require board review of the variance. Ms. Langler further <br /> moved to approve a waiver of the$100.00 camp fee. Motion seconded by Laurel Almquist. VOTE: <br /> Unanimous (3-0), <br /> 4. Request for new Mobile Food Vendor Permit, George & Jack's Simply Grilled Cheese, Mr. <br /> Harrington commented that a new Mobile Food Vendor permit is being requested. The ServSafe <br /> and Allergy Awareness Certificates, as well as the Hawkers & Peddlers License and menu have <br /> been provided, The fee was paid and the inspection was performed with no issues documented. <br /> There being no further comments or discussion, Mallory Langler moved to approve the request <br /> for a new Mobile Food Vendor Permit for George & Jack's Simply Grilled Cheese. Motion <br /> seconded by Laurel Almquist. VOTE: Unanimous(3-0). <br /> 5. Request for new Food Establishment Permit,Castaway Cafe, Mr. Harrington commented that a <br /> new Food Establishment Permit is being requested. The location is the former Pizza Prima <br /> establishment. The ServSafe and Allergen Awareness Certificates,as well as the liability insurance <br /> and menu have been provided. The fee was paid and a preliminary inspection was performed <br /> with no issues documented. Several updates to the facility have been made. <br /> There being no further comments or discussion, Mallory Langler moved to approve the request <br /> for a new Food Establishment Permit for Castaway Cafe with the condition that a satisfactory <br /> inspection is conducted prior to issuance of the permit. Motion seconded by Laurel Almquist, <br /> VOTE: Unanimous(3-0). <br /> 6. Request for one-time Temporary Food Permit fee, Wolf Pizza, Mr. Harrington commented that <br /> a one-time annual Temporary Food Permit fee is being requested. The food operator does not <br /> have a mobile food cart or trailer, but sets up a tent with a wood-fired pizza oven. Inspections <br /> will be conducted at each event. <br /> There being no further comments or discussion, Laurel Almquist moved to approve the one-time <br /> annual Temporary Food Permit fee for Wolf Pizza on the condition that the operator comply with <br /> 4 <br />
