• <br /> MINUTES March 5. 1992 BOARD of HEALTH <br /> APPOINTMENT: Cape & Islands - 13 Cordwood Road <br /> Mr. John s lavinsky, representing the firm of Cape & Is- <br /> lands Engineering and Ms. Lisa Fiori , owner of #13 Cordwood <br /> Road, were in attendance for this scheduled appointment. <br /> Mr, sl.arinsky presented certified mail receipts to the <br /> Board. <br /> Ms. Fiori informed the Board at this time she isfinan- <br /> cially unable to hook into public water. <br /> Mr, Greelish relayed he would be agreeable to the vari- <br /> ance requests with the stipulation Ms. Fiori agree to tie <br /> into public water upon development of the property or trans- <br /> fer of property. <br /> Mr. Greelish motioned to grant a variance from abutting <br /> well , 9 Cordwood load) , to on-site ' septic with the stipula- <br /> tion the Zoning Board of Appeals forwards a letter to this <br /> office stating the lot meets current zoning requirements,, the <br /> Building Inspector* "signs-offs' on the building permit and the <br /> owner submits a statement agreeing to hook 'into public water,," <br /> upon transfer of property' or de lopmt nt of k,.the ?lot,, Mr. r <br /> Costa seconded, all agreed. <br /> APPOINTMENT: Ca e & Islands' - 12 Uncatena Road <br /> Mr , John sl.avinsky, representing the firm of Cape & Is- <br /> lands Engineering was. inµ*attendance for thi lrsche duled •ap- <br /> e <br /> • <br /> po 1 ntme nt. + <br /> - F <br /> Y s # <br /> Mr , Slavinsky presented certified mail receipts to the <br /> Board ;and stated the existing system consists of a septic <br /> tank and cesspool . 'The owner has indicated the leachingfa- <br /> cility on occasion does not function properly and would like <br /> to upgrade. " "The proposed upgrade will require a 2611 vari- <br /> ance from abutting well, Uncatena Road) , and a 251 vari- <br /> ance from abutting well , (#9 Uncatena Road) . <br /> Mr. Greelish motioned to grant a 261 variance from abut- <br /> ting well, (#5 Uncatena Road) , to on-site septic and a 251 <br /> variance from abutting well , (#9 Uncatena Road) , to on-site <br /> septic, Mr. Costas seconded, all agreed4b <br /> APPOINTMENT: Diane Cloutier - Ma Glockner's <br /> Ms, Diane Cloutier and Mr, Michael Hennessey were in at- <br /> tendance for this scheduled appointment. <br />
