t ' <br /> f <br /> MINUTES November 19, 1992 3 'BOARD OF HEALTH <br /> Mr , Doherty motioned to approve plans as presented with <br /> the stipulation thengine r � evise., the tank size to o <br /> gallons and the engineer advisee owner- to install a re- <br /> serve pit alone with the primary, Mr. Ball seconded, all <br /> agreed. <br /> APPOINTMENT: Kathleen Dawkins - 20 Noisy Hole Rd.- <br /> Ms. <br /> id.-Nis. Dawkins did not attend the meeting as requested. <br /> Mr. McQuaid relayed, on November 4 , 1992 he received an <br /> anonymous call regarding illegally dumped materia. off Meet- <br /> inghouse Road. "I inspected the area and. found a large cyan- <br /> tity of illegally dumped household trash most of which had <br /> Ms. Dawkins name on it, " "I forwarded a letter to Ms. <br /> Dawkins which requested her attendance at this meeting to <br /> discuss the matter, failure to do so would result in a <br /> $300,00 fine.. " "The letter was sent certified mail however, <br /> the return receipt was never received. " <br /> Mr. Gr elfish motioned to issue a $300.00 citation to Ms, <br /> Kathleen Dawkins, Mr. Ball seconded, all agreed. <br /> APPOINTMENT: Michael McGrath - 7 Beach }load <br /> Mr-. Michael McGrath was in attendance for this scheduled <br /> appointment. <br /> Mr, McGrath presented revised plans to' the Board and <br /> stated, "upon inspection of the existing septic .s rster on the <br /> lot it was apparent the stone was disrupted during installa- <br /> tion of the leaching facility on the adjacent lot" . <br /> Mr. McQuaid commented, "originally the tank was located <br /> within the easement and this Board required the engineer and <br /> installer to relocate the tank onto the Solomen property. " <br /> tit that time it was my opinion that during relocation of the <br /> system the stone at the edge of the flow diffuser was dam- <br /> aged. 11 "Questions have now been raised as to whether theWa- <br /> ter District may have disrupted the stone while installing <br /> the line- " <br /> Mr. McGrath commented, "when -he inspected the system <br /> there was water at the bottom of the flow diffusers. " "I did <br /> advise the owner that through use and physical changes the <br /> system would have a shorter life- " "Initially I did propose <br /> a repair to the system however, the owner requested the addi- <br /> tion of a reserve which would be located on the lot itself. 11 <br /> "A pert test was performed at which time the ground water <br /> level, was found to be higher than what it was in 1978. 11 "We <br />
