There being no further comments or discussion,Mallory Langler moved to approve the 2019 Food <br /> Establishment Permit for Little Shop of Olive Oils with the following conditions: 1) a dedicated <br /> hand sink is provided; 3) a Food Protection Safety Manager Certificate is provided; 2) a final <br /> inspection is performed prior to operation and issuance of the permit. Motion seconded by Laurel <br /> Almquist. VOTE: Unanimous (3-0). <br /> 4. Request for new Food Establishment Permit, Ocean Potion Smoothies&Doughnuts,259 Shore <br /> Drive, Mr. Harrington commented that the application, permit fee, menu, floor plan, Allergen <br /> Awareness and ServSafe Certifications have been provided. One inspection was performed but <br /> the hot water was not to temp. The applicant is working on correcting that, The doughnuts come <br /> from a bakery in Rhode Island;therefore, no doughnut preparation will be performed on-site. <br /> There being no further comments or discussion,Laurel Almquist moved to approve the 2019 Food <br /> Establishment Permit for Ocean Potion Smoothies & Doughnuts on the condition that a final <br /> inspection is performed prior to operation and issuance of the permit, Motion seconded by <br /> Mallory Langler. Request for new Mobile Food Vendor Permit, Mom and Pops Burgers Food Truck, Mr. <br /> Harrington commented that the application, permit fee, menu,Allergen Awareness and ServSafe <br /> Certifications, State Hawkers & Peddlers License, Town of Chatham Mobile Food Permit, and <br /> proof of workers compensation insurance have been provided. The final inspection will occur on <br /> July 2,2019 for an event at Naukabout Brewery. <br /> There being no further comments or discussion, Mallory Langler moved to approve the 2019 <br /> Mobile Food Vendor Permit for Mom and Pops Burgers Food Truck on the condition that a final <br /> inspection is performed prior to operation and issuance of the permit. Request for variance from the Mashpee Board of Health Bodywork Therapies Regulation,Ann <br /> Paparella, Mr. Harrington commented that the applicant would like to visit homes and <br /> companies to perform reflexology. The local regulation requires that bodywork must be <br /> performed at a single licensed physical location. If bodywork is requested to be performed away <br /> from the licensed facility then a variance is required. For reflexology, a facility and practitioner <br /> license are required. CORI's and SORI's are also required by regulation. Our office has received <br /> no complaints on the applicant,who has been practicing for many years. <br /> There being no further comments or discussion, Mallory Langler moved to approve the 2019 <br /> Bodyworks Practitioner Permit, with a variance to allow the practitioner, Ann Paparella, to <br /> practice at private properties on the condition that an inspection of the facility be performed,and <br /> passing CORIs and SORIs are submitted prior to operation and issuance of the permit. Motion <br /> seconded by Laurel Almquist, VOTE: Unanimous(3-0). <br /> 7. Review and discussion of Nitrogen Aggregation Plan, 232 Cotuit Road. Mr. Harrington <br /> commented that revised plans and Grant of Title V Nitrogen Loading Restriction and Easements <br /> have been submitted, because the land surveyor noticed that there was a taking of land on the <br /> credit property that affected the credit area. <br /> There being no further comments or discussion, Mallory Langler moved to approve the amended <br /> Nitrogen Aggregation Plan for 232 Cotuit Road, as shown on the plans prepared by Cape and <br /> 6 <br />
