Site Plan Review, July 21, 2004 <br /> Page 3 <br /> 131 Industrial Drive <br /> All structures are to be classified in a use group with respect to occupancy. Tom <br /> suggested that they file with the ZBA under Section 174-24.C. (Special permit use) (9) <br /> modify the original special permit— (a) minor changes to the site plan of the permitted 4 <br /> project, such as landscaping, signage, location of buildings, architectural details, lot lines <br /> or roadway facilities. <br /> Needed: 2-way egress to get out; fire alarm common system; block part of driveway <br /> leading to back and add 5 more parking spaces to the existing II for a total of 16 (see <br /> engineer for site plan change); water and septic useage reports to Board of Health and <br /> DEP; To file with the ZBA under Sect. 174-24.C. (9)(a) per Tom Fudala. Need to define <br /> use group, need to modify site plan with ZBA. <br /> Sava Tree (lawn/tree care) <br /> 100 Echo Road <br /> Paul Deptula presenting <br /> Considering moving from Cotuit to Mashpee. Looking for storage and office space. Bulk <br /> liquid fertilizer to be stored in plastic tank. Conditions of approval 220 liquid and 200 dry. <br /> Secondary tank containment recommended by the deputy fire chief but not required by <br /> State since tanks are inside. There is a berm running around the facility outside. <br /> Russ looked over the special permit on the property and found that the permit was for <br /> Frank Maki giving him permission to operate a concrete precasting business, for the sale <br /> of burial vaults, also sold are gravestones and monuments as well as grave digging <br /> services. It would be necessary to get a special permit for the new use that Mr. Deptula <br /> suggested. <br /> Needed: ZBA special permit; new site plan showing the existing buildings. Suggested <br /> make your lease contingent upon ZBA approval. <br /> Mashpee Commons <br /> Use permits and fit-ups <br /> John Renz and Tom Ferranti <br /> They wish to clear up occupancy permits for each new tenant that comes into the <br /> Commons. Each tenant must apply to the building department for a building permit to <br /> alter that space or use permit to occupy the space. Board of Health and Fire <br /> Departments are required to sign off on all applications. A final inspection will be done <br /> and a certificate of occupancy will be issued for each tenant. <br /> Needed: Update of new tenants to Building and Fire Departments (space # and new <br /> street#); send tenant to Building Department with original plan; they need permits for <br /> any updating of carpet, paint, sprinkler system, fire protection and alarm and other <br /> renovations being preformed by tenants. Update parking spaces and handicap signage <br /> to warn, "Tow at owner's expense." <br />
