153 Industrial Drive <br /> Co-located antennas and ground equipment on existing tower <br /> Attorney Carl Gehring,Representing Bell Atlantic MobileNerizon Wireless <br /> They want to place their antenna at the 143' spot below the top and place a shelter at the bottom. <br /> 8 tenants there now, and they are the 9`t'. They want to install a prefab shelter 1200 on a slab to <br /> house radio equipment and a standby generator which is totally enclosed in the building. They <br /> plan to bump the fence line 14' on this large parcel. There are no setback issues. Fire Dept. <br /> recommend that a knox box be placed outside (key)to get into the building. Fire Deputy asked <br /> for written documentation from the Tower owner for an emergency plan of action re: medical <br /> issues and rescue for technicians who fall ill or injure themselves on the tower. A technical <br /> rescue team might be required. There is probably a tower safety plan that is standard. The <br /> attorney will check into this and give the information to Pat in the Building Office. They need a <br /> building permit for the prefab along with drawings. <br /> The Special permit conditions (6 and 7) requires RFR noise levels every three years. They need <br /> to find out if this was done. <br /> They need minor modification approval from ZBA since they are making changes to the site <br /> plan per By-law 174-24.C9 page 19 Minor Change to Site Plan. They will sign up with the <br /> ZBA, but no public hearing will be necessary. Pat White will provide the paperwork needed. <br /> They will need to check in with Cape Cod Commission 362-3828 since it is part of the <br /> Development Agreement between the CCC and Industrial Park. The tower is over 30' and <br /> requires review by the CCC Chief Regulatory Officer. The attorney requested the written <br /> Development Agreement with conditions and rules, and Tom Fudala will provide one for him to <br /> review. They need to make sure the shed is in conformance. <br /> Summary: <br /> No requirements from the Board of Health <br /> Apply to ZBA for modification to the plan <br /> Provide details on the shelter to the Building Dept. <br /> Provide protocol for emergency situation on tower <br /> The following Minutes were moved, seconded, and unanimously approved as amended: <br /> November 12, 2004,December 10, 2004, January 14, 2005, and March 25, 2005. <br /> Next meeting will be announced as needed. <br /> Adjourned 10:40 a.m. <br /> Mary Ge sthorpe,Recording Secretary <br />
