Page 2 <br /> Capital bnprovevvent Requests <br /> Please note that a capital request must be submitted for all capital outlays,even those <br /> which were approved under last year's program,but still require funding.A"capital <br /> outlay"is a major,non-routine expenditure for new construction,major equipment <br /> purchase,or improvement to existing buildings,facilities,land or infrastructure,with an <br /> estimated useful life of five(5)years or more,and a cost of$25,000 or more. <br /> Among the items properly classified as capital improvements are: <br /> • New public buildings,or additions to existing buildings,including land <br /> acquisition costs and equipment needed to furnish the new building or addition <br /> for the first time; <br /> • Major alterations,renovations,or improvements to existing buildings which <br /> extend the useful life of the existing buildings by five(5)years; <br /> • Land acquisition and/or improvement,unrelated to a public building,but <br /> necessary for conservation or parks and recreation purposes; <br /> • Major,equipment acquisition,replacement or refurbishment,with a cost of at <br /> least$25,000,and a useful life of at least five(5)years; <br /> • New construction or major improvements to the Town's physical infrastructure, <br /> including streets,sidewalks,stormwater drains,the water distribution system,and <br /> the sanitary sewer system,which extend the useful life of the infrastructure by at <br /> least five(5)years;and <br /> • A feasibility study or engineering design services which are related to a future <br /> capital improvement. <br /> A separate Project Request Forn is required for each capital outlay,and a single <br /> Summary Form is required to list all projects requested.As an example,if you are <br /> requesting three separate capital outlays,you should submit three individual Project <br /> Request Forms and one single Summary Form. <br /> Please note that the deadline,for completing all request forms and submitting theft?to <br /> my office is IF'etJrresrbt€r;,I�ecernbea }.2(l.T d'.
