AMENDMENT NO. 3 <br /> TOWN OF MASHPEE, MA AND <br /> GHD INC. <br /> PAGE 5 <br /> 2.2 Permitting <br /> A. 'Provide technical criteria, written descriptions, and design data for Town's use in filing <br /> applications for permits from or approvals of governmental authorities having jurisdiction to <br /> review or approve the final design of the Project. Assist Town in consultations with such <br /> authorities and revise the Drawings and Specifications in response to directives from such <br /> authorities. The following permits are anticipated to be submitted as part of this contract: <br /> 1. SRF contract document submission (including Plan Review by DEP) based on one <br /> submittal. <br /> 2. All other permitting is assumed to be prepared by the Town's Transportation Engineer. <br /> 2.3 Field Investigations <br /> A. Survey for sewer infrastructure is not included in this scope. Provide recommendations to <br /> the Town and the Town's Transportation Engineering Consultant on additional survey work <br /> required to complete the sewer design. Town Transportation Engineering Consultant to <br /> coordinate all additional survey work.Additional survey that might be anticipated is the <br /> establishment of first floor elevations on buildings and properties and additional topography <br /> or utility information not already captured on the Transportation Engineering Consultant's <br /> drawings in areas where sewers may be proposed. <br /> B. Borings <br /> 1. Soil borings are not included in this Contract. Review subsurface exploration findings <br /> already conducted for the road restoration design(to be provided by the Town). Provide <br /> recommendations on additional subsurface exploration with borings required to complete <br /> the sewer design. Town Transportation Engineering Consultant to coordinate all <br /> additional boring work. <br /> TOWN RESPONSIBILITIES <br /> 1. Provide GHD with all criteria and full information as to Town's requirements for the Project, including design <br /> objectives and constraints, space, capacity and performance requirements, flexibility and expandability, and any <br /> budgetary limitations; furnish copies of all design and construction standards which Town will require to be <br /> included in the Drawings and Specifications; and furnish copies of Town's standard forms, conditions,and related <br /> documents for GHD to include in the Bidding Documents,when applicable. <br /> 2. Furnish to GHD any other available information pertinent to the Project including reports and data relative to <br /> previous designs or investigation at or adjacent to the Site. <br /> 3. Arrange for safe access to and make all provisions for GHD to enter upon public and private property as required <br /> for GHD to perform services under the Agreement. <br /> 4. Examine all alternate solutions, studies, reports, sketches, drawings, specifications, proposals, and other <br /> documents presented by GHD(including obtaining advice of an attorney, insurance counselor,and other advisors <br /> or consultants as Town deems appropriate with respect to such examination)and render in writing timely-decisions <br /> pertaining thereto. <br /> 5.
