The stormwater management form at the end of the report has not <br /> been signed or dated. <br /> III. Plans <br /> A. Sheet S-1 <br /> 1. Much of the dimensioning along the proposed road sidelines is <br /> unreadable and should be made larger. <br /> 2. Is this plan to be recorded in the Barnstable Registry of Deeds? If <br /> so a proper registry block must be provided as well as a signature <br /> and date of approval block. <br /> 3. Provide a block for date of approval and Board signatures. <br /> 4. Will this plan be used to rescind the layout of Lettie Lane as <br /> shown on the Land Court plan? <br /> 5. All proposed new lot lines and dimensioning should be shown. <br /> 6. It is recommended that street line monuments should be shown <br /> on this plan to define the street location. It will only assist future <br /> homeowners of the project in determining property boundaries <br /> within reasonable cost. <br /> 7. It is recommended that all site construction details be placed on a <br /> separate plan from the one that defines the street lines and lot <br /> lines. <br /> 8. This plan indicates road baseline stationing commencing at two <br /> unknown points within Route 130 baseline. These starting points <br /> need to be identified with respect to the sideline of Route 130 and <br /> the existing monumentation shown on the plan. <br /> 9. The proposed roadway data indicates a waiver is requested for <br /> the minimum curb radius to be changed from 35' to 25'. Why is <br /> this waiver needed? <br /> 10. The pavement width is indicated as 20 feet and this dimension is <br /> in compliance with the Mashpee Subdivision Rules and <br /> Regulations for general subdivision streets with grades less than <br /> 3%. It is recommended that 18" berms be included for a total <br /> street pavment width of 23 feet in order to properly control surface <br /> runoff within the paved area and to prevent shoulder erosion. <br /> 11. A waiver for the source of the topography shown on the plan has <br /> been requested. A statement as to the accuracy of the <br /> topography should be provided and a suitable benchmark should <br /> be shown on the plan. Because of the significant changes in <br /> elevation and the amount of material to be regraded, accurate <br /> topography will be vital to a successful project. <br /> 12. No rim or pipe invert elevations are noted on the plans. No <br /> stationing or distances right or left of the road baseline are noted <br /> for the catch basins or manholes shown. No pipe sizes or types of <br /> material are noted. <br /> 13. Catch basins are shown but with no connection to the street <br /> pavement. How does surface runoff get to these basins? What <br /> type of frame and grate are provided? <br />
