3. In preparing final construction plans for approval by the Board's engineer, the <br /> Applicant must address the concerns and recommendations of the Board's <br /> engineer, as noted in the attached exhibit. Particularly, the Applicant must: <br /> a. provide concrete bounds; <br /> b. provide "dimensional ties;" <br /> C. define the drainage area(Lot 5) with appropriate metes, bounds and area; <br /> d. clarify the reference to "T.F.;" <br /> e. investigate the function and design of the existing town drainage structure <br /> that is located within the layout of the proposed roadway. It is noted by the <br /> Board's engineer that this structure may have to be relocated or redesigned <br /> at the Applicant's expense. The Applicant should coordinate its efforts in <br /> this regard with the Board's engineer and the Town's Public Works <br /> Department; I <br /> £ redesign (by widening or otherwise)the common driveway for Lots 1 and 2 <br /> such that safe access can be assured; <br /> g. provide assurances (by indicating a berm or otherwise) that runoff can be <br /> controlled on the stub roadway between Lots 8 and 9; <br /> h. address the Board's engineer's comments with respect to stormwater <br /> management facilities on lots 4 and 5 so as to allow for proper and efficient <br /> drainage and minimization of impacts to the septic system on Lot 2; <br /> i. provide a minimum binder course of 1-3/4" and a minimum top course of 1- <br /> 1/4". <br /> j. comply with the Mashpee Water District's requirements with respect to <br /> minimum depth of cover over water pipes; I <br /> k. comply with the Board's engineer's recommendations regarding catch basin <br /> design; <br /> is <br /> 1. obtain the prior written approval of the Mashpee Fire Department for all !, <br /> hydrant locations; j <br /> in. confirm soil conditions for proper design of stormwater management <br /> system. Results of soil testing should be provided to the Board's engineer; <br /> n. the maximum side-slopes of road shoulders should be 2:1 and should be <br /> noted on the plans; <br /> o. provide additional information with respect to the infiltration system located <br /> on Lot 12. a <br /> 4. The Applicant shall submit to the Board of Appeals a definitive and final plan, <br /> suitable for endorsement by the Board and recording at the Barnstable County <br /> Registry of Deeds, showing the layout of roadways and the configuration of house !' <br /> lots and open spaces properly labeled for their endorsement prior to conveyance I <br /> of any portion of property subject to this comprehensive permit. The <br /> endorsement"block" shall not make reference to the subdivision control law and, <br /> instead, shall include the following notation: "Approved and endorsed under G.L. <br /> c. 40B by the Mashpee Zoning Board of Appeals." <br /> i <br /> 5. The Applicant shall comply with all the requirements set forth in the Order of j <br /> Conditions issued by the Mashpee Conservation Commission. In the event that a <br /> modification of such Order of Conditions is necessary in order to widen the <br /> common driveway serving Lots 1 and 2, the Applicant shall diligently apply for <br /> 4 <br />
