r. <br /> JURISDICTIONAL DETERMINATION APPLICATION <br /> Attach the following additional materials: <br /> 1. Two copies of all documents and analysis supporting the request for a Jurisdictional <br /> Determination. The Commission will review your application and notify you by mall <br /> of your public hearing on this application upon a determination that this application is <br /> complete. <br /> 2. Two copies of the application(s) for any development permit(s) filed with Municipal <br /> Agency(ies) together with two copies of: (1) documents supporting such application <br /> which the Applicant or Municipal Agency deems relevant to the Commission's <br /> Jurisdictional Determination, and (2) documentary evidence that such application was <br /> in fact filed. including the date of filing. <br /> 3. Enclose a filing fee of$200 in the form of a money order or certified check made payable <br /> to the Barnstable County Treasurer. <br /> 4. Certification that a copy of this application has been filed concurrently with the town <br /> clerk, the inspector of buildings, and the municipal agency(ies) before which a permit <br /> application is pending, in the town(s) in which the proposed development is to be <br /> located. <br /> 5. A self-addressed stamped envelope, and three blank stamped envelopes for each to%vn in <br /> which the proposed development is to be located, which the Commission shall use to <br /> inform the applicant and municipal agencies when the application is deemed fully <br /> complete and has been accepted by the Commission. <br /> 6. Certification that the subject property is owned by the Applicant or his or her designee. <br /> 7. Two copies of an 8 1/2 "x 11" section U.S.C.S. quadrangle map of the area, containing <br /> sufficient information for the Commission to locate the site of the proposed <br /> development. <br /> S. (OPTIONAL) A suggested form of notice of the Commission public hearing on this <br /> application which fulfills the requirements of Section 5(e) of the Act regarding notice <br /> contents. <br /> I hereby certify that the answers contained in and information attached to this <br /> application form are true and accurate to the best of my knowledge, under penalty of the law. <br /> William J.
