Application forms are to be completed in full and filed and two copies stamped in the office of the Town Clerk. 2. Each Application must be accompanied by a filing fee of $100, payable by check to the Town of Mashpee. 3. Each Application must be accompanied by two sets of plans prepared and signed by a certified engineer or surveyor showing: a. the dimensions of the land b. the exact location of all existing and/or proposed buildings on the land c. drainage, utilities .1acluding septic plans and paved surfaces d. all setback distances of structures from the street, lot lines, wetlands, water body or watercourse with proper identification e. calculation of percent of lot coverage of present and proposed structures f. parking spaces, aisles dimensions, and flow of traffic noted by arrows should be included on commercial Applications g. zoning and current uses of the site and adjacent properties 4. When the Applicant is not the owner of the property the Applicant shall provide the Board with proof of legal interest in the property i.e. agent, trustee, leaseholder or pending purchase and sale agreement. S. It is the responsibility of the Applicant to submit additional copies of the Application and plans to other Town Boards as required in the Zoning By-laws. These plans should be submitted at the same time the Application is filed with the Town Clerk. For example: a. Conservation Commission - for all Applications involving land within 100 feet of a wetland, water body or watercourse b. Shellfish Commission, Harbormaster and Waterways - for all Applications for fixed and floating piers, wharves, docks and boardwalks.
