J,g <br /> 00 <br /> BOARD OF APPEALS <br /> PROCEDURES FOR FILING APPLICATIONS FOR <br /> SPECIAL PERMITS, VARIANCES OR APPEALS <br /> CHAPTER 4OA <br /> APPLICATIONS: <br /> 1. Application forms are to be completed in full and filed and two copies <br /> stamped in the office of the Town Clerk. <br /> 2. Each Application must be accompanied by a filing fee of $100, <br /> payable by check to the Town of Mashpee. <br /> 3. Each Application must be accompanied by two sets of plans <br /> prepared and signed by a certified engineer or surveyor showing: <br /> a. the dimensions of the land <br /> b. the exact location of all existing and/or proposed <br /> buildings on the land <br /> c. drainage, utilities .1acluding septic plans and paved surfaces <br /> d. all setback distances of structures from the street, <br /> lot lines, wetlands, water body or watercourse with <br /> proper identification <br /> e. calculation of percent of lot coverage of present and <br /> proposed structures <br /> f. parking spaces, aisles dimensions, and flow of traffic noted <br /> by arrows should be included on commercial Applications <br /> g. zoning and current uses of the site and adjacent properties <br /> 4. When the Applicant is not the owner of the property the Applicant <br /> shall provide the Board with proof of legal interest in the <br /> property i.e. agent, trustee, leaseholder or pending purchase and <br /> sale agreement. <br /> S. It is the responsibility of the Applicant to submit additional <br /> copies of the Application and plans to other Town Boards as <br /> required in the Zoning By-laws. These plans should be submitted <br /> at the same time the Application is filed with the Town Clerk. <br /> For example: <br /> a. Conservation Commission - for all Applications involving land <br /> within 100 feet of a wetland, water body or watercourse <br /> b. Shellfish Commission, Harbormaster and Waterways - for all <br /> Applications for fixed and floating piers, wharves, docks <br /> and boardwalks. <br />
