This revision would include in the Mashpee Septic Inspection regulations that if the report and <br />payment are not submitted within the 30 days of inspection then it is not completed and the <br />report would not be approved, requiring a new inspection and report to be completed. <br />Mr. Seabury answered board members questions pertaining to title 5 reports. The report must be <br />submitted 30 days from the inspection date. As of right now, a passing title 5 report is $25, while <br />a conditional pass or fail is $100. Mr. Seabury stated that he is looking to change that in the future <br />with a flat rate to review the report. <br />There being no further comments or discussion, Ernest Virgilio motioned to approve the Septic <br />Inspection Regulation Amendment. Mr. Seabury commented it was recently brought to the attention of <br />Health Agents that Septic Designers in the town of Mashpee have not been submitting an <br />Engineered AS -built. <br />Within 30 days of the final inspection of the system and prior to the issuance of a Certificate of <br />Compliance, the Disposal System Installer and the Designer shall certify in writing on a form <br />approved by the Department that the system has been constructed in compliance with 310 CMR <br />15.000, the approved design plans and all local requirements, and that any changes to the <br />design plans have been reflected on as -built plans which have been submitted to the Approving <br />Authority by the Designer prior to the issuance of a Certificate of Compliance. <br />The as -built plans shall be prepared in accordance with 310 CMR 15.210 and, at a minimum, <br />shall reflect the following: <br />1. any changes to the approved design plans and <br />2. show the exact location and <br />3. elevation of all system components. <br />There being no further comments or discussion, John Livingston motioned to acknowledge the <br />state regulation requiring an as -built card from the engineer prior to the issuance of the <br />Certification of Compliance. Mr. Seabury commented there was a survey done by the <br />Planning department, a Local Comprehensive Plan for all Mashpee residents to take. It is on the <br />Town's website. It is a 20 minute survey for residents to offer their opinion on certain things in <br />the Town and what they would like to see in the Town. Mr. Seabury wanted to bring in to the <br />Boards attention. It is a good opportunity for the public to tell the town what it wants, needs and <br />likes. <br />7. Time change for Board of Health meetings. Mr. Virgilio commented he would like to change the <br />time of the Board of Health meetings to earlier. He would prefer 4 PM but mentioned that Mr. <br />Seabury would prefer 4:30 PM. Mr. Livingston brought up a possible day change as well, maybe <br />Tuesdays to stay away from long weekends. <br />There being no further comments or discussion John Livingston motioned to approved the change <br />of time the Board of Health meetings to 4:30 PM, and to the 1' and 3rd Tuesday pending approval <br />of the Clerk's office.
