760ue Castle Drive—Final Inspection on2-1-21,As-built uploaded on8-12-2I <br /> 80 Blue Castle Drive—Final Inspection on 4-22-22, No As-built yet <br /> 16 Carriage Road — Hna| inspection on December 13m 2022, xvhen the Department inspector <br /> went it was found that the system installed was not the system that was designed. System was <br /> removed and installed a second time which met the Leaching field design. Health Agent found <br /> that a second line tothe Septic Tank from the foundation was added without contacting the <br /> Health Department or the Design engineers,this new line also crossed the water line. When the <br /> Health Agent asked Mr. Morin why this was added,through email, Mr. Morin never replied. <br /> Mr. Morin commented that 58 Blue Castle, he put in what was on the original and admitted that <br /> he does not have a good reason why the as-bui|ts are not supplied in atirne|y manner. He <br /> promised to upload the as-bui|ts going forward. <br /> Mr. Livingston commented that he runs a business in Mashpee and understands the regulations <br /> that his business needs to adhere to and stated that the list is pretty egregious and needs to be <br /> addressed right away. <br /> Mr. Seabury commented this problem happens constantly with this contractor not completing <br /> the required as-bui|ts, he put in a completely wrong system then what the plan said and when <br /> the health agent reached out to Mr. Morin there was no response from Mr. Morin. <br /> Mr. Seabury recommended giving Mr. Morin a warning, and if this issue happens again that the <br /> department can revoke his license to install in Mashpee for disregard to Title 5regu|ations and <br /> lack wf communication with the office. <br /> Mr. Livingston made a motion togive Mr' Morin a final notice, if there is one violation within the <br /> next year the Board will suspend his license for the rest of the year. Mr. Rapoza does not second <br /> the complete motion. He stated that if there is another problem Mr. Morin should come in front <br /> of the board again to address the problem. He does not agree to suspend Mr. Morin's license. <br /> Mr. Rapoza made a motion that forfuture problems, Mr. Morin go in front of the Board to explain <br /> what went wrong. Mr. Livingston withdrew his motion and seconded Mr. Rapoza's motion. <br /> Motion was approved unanimously. Roll Call Vote: (2-O) <br /> Mr. V|rgi|io returned to the meeting. <br /> Show Cause, Board of Health Order—Matthew Haney <br /> Mr. Seabury commented Per M.G.L c. 140, 32G,the Town of Mashpee,through its Board of <br /> Health is to collect License Fees from the MEZ Realty for the manufactured housing community <br /> known as Lakeside Estates, along with the fee a list containing the names and addresses of each <br />
