MINUTES March 4, 1993 3 BOARD OF HEALTH <br /> well, ( lot 20 Cayuga Avenue) , to the proposed on-site leach- <br /> ing facility, a 21' local variance from abutting well, (lot <br /> 12 Cayuga Avenue) , to the on-site leaching facility and a 27' <br /> local variance from abutting well, (lot 18 Cayuga Avenue) , to <br /> the proposed on-site leaching facility, Mr. Doherty seconded, <br /> all agreed. <br /> APPOINTMENT: Brian Ayotte - License suspension <br /> Mr. Ayotte was in attendance for this scheduled appoint- <br /> ment. <br /> Mr. Ayotte stated, "Last week this Board suspended my <br /> license for 30 days without knowing the complete story. " "I <br /> went to the job, cut down approximately 5 trees and dug the <br /> hole for the 1500 gallon septic tank. " "I then came to the <br /> Board of Health and requested a septic permit and was in- <br /> formed plans were not submitted. " "The Real Estate company <br /> has taken full responsibility as they were to submit plans, <br /> water test, etc. " "The tank was ordered, all I did was drop <br /> it in the ground." <br /> Mr. Doherty stated, "Unfortunately, the Real Estate com- <br /> pany is not the one holding the installer's permit and they <br /> did not dig the hole. " <br /> Mr. Greelish informed Mr. Ayotte that plans finally sub- <br /> mitted were not approved. "No work should have proceeded un- <br /> til plans were approved. " <br /> Mr. McQuaid informed Mr. Ayotte, "The concern the Board <br /> had with the plan was not the location of the tank. " "It may <br /> have been the location of the pit as we did not have exact <br /> distances from abutting wells. " "I did witness the tank in <br /> the ground however, no piping was connected to or from it. " <br /> Mr. Doherty motioned to rescind Mr. Ayotte's 30 day li- <br /> cense suspension, Mr. Ball seconded, all agreed. <br /> APPOINTMENT: John Slavinsky - Cardillo -- Pimlico Pond Rd. <br /> Mr. John Slavinsky, representing the firm of Cape & Is- <br /> lands Engineering, was in attendance for this scheduled ap- <br /> pointment. <br /> Mr. Slavinsky presented plans and relayed, "We are pro- <br /> posing to pump and fill the existing cesspools as neither one <br /> can make break-out. " <br />
