r. y <br /> MINUTES May 27 ► 1993 3 BOARD OF HEALTH <br /> Agent of the Board and compliance with the town of Mashpee <br /> "Refuse Regulation" , Mr. Ball seconded, all agreed. <br /> 5. ) Bi-monthly inspection report from Weston & Sampson <br /> LANDFILL/TRANSFER STATION INSPECTION - Acknowledged <br /> LAGOON INSPECTION - Acknowledged. <br /> 6 . ) Correspondence re: Septic repair, 41.1_Monomoscoy Island <br /> Road - Mr. Evans relayed, "A complaint was received regard- <br /> ing a broken#sewer line. " "Upon the initial inspection I was <br /> unable to locate the' broke line however, when a second com- <br /> plaint was received by the same individual we met at the site <br /> and he directed me to where the broken line was located. " <br /> "Inspection revealed a broken line covered by a tin can with <br /> branches over the top and sides. " "Subsequently, I notified <br /> the owner of the property and advised him the system must be <br /> repaired. " "The owner responded in writing that a temporary <br /> repair was made to preclude any possibility of a health haz- <br /> ard. " "As there is no sign of failure I am requesting the <br /> Board's opinion as to whether the entire system should be re- <br /> placed or just the one line. " "In this case the plumbing in- <br /> spector has juristiction as the drain plumbing extends less <br /> than ten feet from the structure. " <br /> Mr. Greelish relayed, "I did inspect the property and <br /> damage to the pipe. " "The system as it exists does not meet <br /> Title V requirements. " "I would recommend we request the <br /> owners to attend an upcoming meeting to discuss the matter. " <br /> Mr. Doherty directed Mr. McQuaid to forward a letter to <br /> the owner expressing this Board's concern that the system was <br /> repaired without a repair permit and since this incidence has <br /> been brought to our attention we will require an evaluation <br /> of the system. " <br /> 7. ) Emergency failure certification 4 Degrasse Road - Mr. <br /> Evans relayed, "I inspected the site and found the system to <br /> be in failure. " "As of yet the installer has not submitted <br /> the repair plan. " <br /> Board members forwarded their signatures to the "Emer- <br /> gency Repair Certification" form. <br /> 8. ) Variance request - 7 Ouahog Court and 89 Shields Rd. <br /> 7 QUAHOG COURT - Mr. McQuaid relayed, "This is a preapproved <br /> subdivision." "Plans were approved in 1986 but have since <br /> expired. " "The owner is requesting a 35- local variance from <br /> the proposed on-site septic to the proposed on-site well, a <br /> 8' local variance from the abutting leaching facility, (74B) , <br />
