MINUTES July 29, 1993 2 BOARD OF HEALTH Mr. Greelish motioned to grant an 18' local variance from the proposed on-site leaching facility to the proposed on-site well as requested, Mr. Ball seconded, all agreed. 7. ) Proposed smoking questionnaire - Mr. Evans presented the questionnaire for the Board's approval. w Mr. Greelish motioned to approve the questionnaire as drafted, Mr. Ball seconded, all agreed. APPOINTMENT: Tom Howes - Willowbend Treatment Plant Mr. Tom Howes and Mr. Mark Pare' "was in- attendance for• this scheduled appointment. i Mr. McQuaid informed Mr. Howes, "We did receive a copy of D.E.P. 's approval for the draft discharge permit. " As this treatment plant was proposed prior to the Board of Health adopting a regulation whereby all proposed treatment plants must be reviewed by the town's consulting engineers we are requesting Willowbend voluntarily submit to the review process by our consulting engineers. " Mr. Pare' relayed, "We did receive approval of the plans and specifications from D.E.P. . " Mr. McQuaid questioned Board members whether the dis- charge permit fee should be incorporated with the review fee or whether payment should come from the engineering account. Mr. Greelish proposed that Board members consult with our engineers as to the scope of the project and request they provide an estimate. "Once we receive an estimate we will incorporate it into the overall fee for the discharge permit. " Board members agreed. Mr. Greelish motioned to request the town engineers pro- vide an estimate for review costs, once received the estimate will be incorporated into the overall fee for the discharge permit, Mr. Ball seconded, all agreed. APPOINTMENT: Michael Grotske - New Seabury "Flat Pond" Mr. Grotske, Mr. Dave Worden representing the engineer- ing firm of Frugro-McClellan and Conservation Commission Agent Bob Sherman were in attendance for this scheduled ap- pointment. Mr. Grotske presented copies of the hydrogeological study.
