MINUTES July 29, 1993 2 BOARD OF HEALTH <br /> Mr. Greelish motioned to grant an 18' local variance <br /> from the proposed on-site leaching facility to the proposed <br /> on-site well as requested, Mr. Ball seconded, all agreed. <br /> 7. ) Proposed smoking questionnaire - Mr. Evans presented <br /> the questionnaire for the Board's approval. <br /> w <br /> Mr. Greelish motioned to approve the questionnaire as <br /> drafted, Mr. Ball seconded, all agreed. <br /> APPOINTMENT: Tom Howes - Willowbend Treatment Plant <br /> Mr. Tom Howes and Mr. Mark Pare' "was in- attendance for• <br /> this scheduled appointment. <br /> i <br /> Mr. McQuaid informed Mr. Howes, "We did receive a copy <br /> of D.E.P. 's approval for the draft discharge permit. " As <br /> this treatment plant was proposed prior to the Board of <br /> Health adopting a regulation whereby all proposed treatment <br /> plants must be reviewed by the town's consulting engineers we <br /> are requesting Willowbend voluntarily submit to the review <br /> process by our consulting engineers. " <br /> Mr. Pare' relayed, "We did receive approval of the plans <br /> and specifications from D.E.P. . " <br /> Mr. McQuaid questioned Board members whether the dis- <br /> charge permit fee should be incorporated with the review fee <br /> or whether payment should come from the engineering account. <br /> Mr. Greelish proposed that Board members consult with <br /> our engineers as to the scope of the project and request they <br /> provide an estimate. "Once we receive an estimate we will <br /> incorporate it into the overall fee for the discharge <br /> permit. " <br /> Board members agreed. <br /> Mr. Greelish motioned to request the town engineers pro- <br /> vide an estimate for review costs, once received the estimate <br /> will be incorporated into the overall fee for the discharge <br /> permit, Mr. Ball seconded, all agreed. <br /> APPOINTMENT: Michael Grotske - New Seabury "Flat Pond" <br /> Mr. Grotske, Mr. Dave Worden representing the engineer- <br /> ing firm of Frugro-McClellan and Conservation Commission <br /> Agent Bob Sherman were in attendance for this scheduled ap- <br /> pointment. <br /> Mr. Grotske presented copies of the hydrogeological <br /> study. <br />
