MINUTES April 14 , 1994 2 BOARD OF HEALTH <br /> APPOINTMENT: Cape & Islands Eng. - Hyde subdivision <br /> Mr. John Slavinsky, P.E. was in attendance for this <br /> scheduled appointment. <br /> Mr. Slavinsky presented a three lot subdivision plan and <br /> stated, "All lots meet septic and well requirements. " <br /> Mr. Ball motioned to accept preliminary plans as pre- <br /> sented, Mr. Doherty seconded. <br /> APPOINTMENT: Cape & Islands Eng. - 29 Seaview Avenue <br /> Mr. John Slavinsky, P.E. was in attendance for this <br /> scheduled appointment. <br /> Mr. Slavinsky relayed, "The elevation has changed since <br /> original plans were submitted. " "I have revised plans to <br /> meet the 100' setback requirements from the top of the em- <br /> bankment. " <br /> Mr. Ball motioned to grant a 8' sideline variance pend- <br /> ing revised plans, Mr. Doherty seconded. <br /> NEW - Variance re est 21 ouipRish Road <br /> Mr. John Slavinsky, P.E. was in attendance for this <br /> variance request. <br /> Mr. Slavinsky relayed, "The owner is requesting a 30' <br /> local variance from abutting well , (House #29 Quippish Road) , <br /> to the on-site proposed leaching facility and a 5' State <br /> sideline variance. <br /> Mr. Ball motioned to grant a 20' local variance from <br /> abutting well, (House #29 Quippish Road) , to the on-site pro- <br /> posed leaching facility and a 5' State sideline variance with <br /> the stipulation public water services the dwelling, Mr. <br /> Doherty seconded. <br /> ,APPOINTMENT: Ben Gordon - 99 Waterline Drive South <br /> Mr. Ben Gordon was in attendance for this scheduled ap- <br /> pointment. <br /> Mr. Gordon relayed, "In 1988 the Board of Health granted <br /> a wetlands variance. " "The lot is only 78' wide and ap- <br /> proximately 110' long. " "I would like to construct a dwelling <br /> similar to the one proposed on original plans. " "We cannot <br /> maintain 100' from the edge of the bordering vegetation. " <br />
