MINUTES October 5, 1995 2 BOARD OF HEALTH <br /> APPOINTMENT: John Steven Lebel -Installer's license <br /> Mr. Lebel was in attendance for this scheduled appointment. <br /> Mr. Lebel stated, "I have had an installer's license for the past few years." "I <br /> would like to renew my license." <br /> Mr. Doherty explained, "The reason Mr. Lebel is before us is because the last time <br /> he applied for his license his fee was paid by check which came back with insufficient <br /> funds in his account." "The health agent wanted the Board to determine if Mr. Lebel <br /> could renew his license." <br /> Mr. Lebel stated, "I would like to straighten this matter out and go back to work." <br /> Mr. Ball motioned to issue Mr. John Steven Lebel a 1995 installer's license, Mr. <br /> Cram seconded, all agreed. <br /> 6.) V.N.A. "Family Quest" - Mr. Evans relayed, "This is part of the human services <br /> program I attended on behalf of the Board of Health. "19,300.00 was appropriated at <br /> town meeting to fund this program and the battered spouse program." "This amount will <br /> be added to current appropriations for payment towards the program." <br /> Mr. Doherty stated, "The grant funds ran out so town meeting approved the funds <br /> but it's strictly for the program, there is no money for an administrator so the funds were <br /> placed under our supervision to be dispersed accordingly." "I would recommend the <br /> health agent forward two letters, one to the V.N.A. indicating the program now comes <br /> under Board of Health direction and a letter to Legal Services of Cape Cod indicating the <br /> change in the municipal overseer and request they provide a description of the program <br /> and services provided." <br /> Board members agreed. <br /> 7.) Variance request, 9 Bog River Bend - Mr. Ball relayed the owner is requesting an <br /> 18' local variance from the proposed on-site leaching facility to the proposed on-site <br /> well." <br /> Mr. Ball motioned to grant the 18, local variance as requested, Mr. Cram <br /> seconded, all agreed. <br />
