MINUTES November 2, 1995 3 BOARD OF HEALTH <br /> Mr. Cram motioned to accept the artillery bunker, (Quonset Hut), offered by <br /> Massachusetts Military Reservation at Otis, Mr. Ball seconded, all agreed. <br /> I) Request for commercial sticker extension- S.C. Hofer&: Willowbend <br /> S.C. HOFER DESIGN- Mr. McQuaid relayed, "Ms. Andrea Spaneas is requesting the <br /> Board consider granting a 1996 commercial sticker which would incorporate the <br /> remaining two months of this year." <br /> Mr. Ball motioned to grant a 1996 commercial sticker to Ms. Andrea Spaneas <br /> d/b/a S.C. Hofer Design valid from November 2, 1995 to December 31, 1996, Mr. Cram <br /> seconded, all agreed. <br /> WMLOWBEND - Mr. McQuaid relayed, "Mr. Bruce Besse d/b/a Willowbend Land <br /> Design is requesting the Board validate his 1996 commercial sticker to include the last two <br /> months of this year." <br /> Mr. Ball motioned to grant a 1996 commercial sticker to Mr. Bruce Besse d/b/a <br /> Willowbend Land Design valid from November 2, 1995 to December 31, 1996, Mr. Cram <br /> seconded, all agreed. <br /> 4.) Memo from Town Clerk re: Stickers, Holiday meeting schedule - Acknowledged. <br /> 5.) Clay distribution program, (authorize Agent to sign) - Mr. Cram motioned to <br /> authorize Mr. McQuaid, as acting health agent, to sign the clay distribution program form <br /> on behalf of the Board of Health, Mr. Ball seconded, all agreed. <br /> 6.) Emergency_failure certification, 4 Mohawk Road - Mr. Evans relayed, "The <br /> leaching facility is definitely in failure." <br /> Mr. McQuaid stated, "A young couple recently purchased the property." "The <br /> women is pregnant and they only have one bedroom." "They would like to convert the <br /> garage into another bedroom." "I informed them the system must be inspected to ensure <br /> it is fianctioning properly." "Upon inspection the system failed." "The well is definitely <br /> not 150' away probably not even 100' away." "Public water is available." "We can't <br /> allow them to construct an addition unless they hook into town water or they retain an <br /> engineer to certify that their well is 150' away." <br /> Board members forwarded their signatures to the "Emergency Repair <br /> Certification" form. <br />
