MINUTES November 12, 1998 3 BOARD OF BEALTH <br /> Mr. Doherty motioned to grant a 1999 food service establishment permit to "The <br /> Womans Body Shopp" said permit shall incorporate the months of November and <br /> December 1998, Mr. Ball seconded, all agreed. <br /> Mr. Doherty motioned to authorize the agent to sign-off on the occupancy permit <br /> for The Womans Body Shopp with the stipulation the health agent receives verbal <br /> authorization from D.E.P. for the salon's holding tank, Mr. Ball seconded, all agreed. <br /> 7.) D.E.P. - N.O.R. Mike's Service Station - Mr. McQuaid relayed, "Mr. DiMaggio <br /> had his tanks excavated." "He had a licensed tank removal outfit excavate the tanks and <br /> there was slightly more than what was expected as far as ground contamination from <br /> overfill on the tanks." "He had an M.C.P. on-site and a licensed tank removal outfit on- <br /> site." "The licensed site professional had the operator dig down to below where the tanks <br /> were and we had the soils tested." "They showed 240 p.p.b. of hydrocarbons." "They <br /> continued to go down hoping to get clean sand so they could excavate the dirty sand_" <br /> "They went down 25' at which time it was beginning to undermine the foundation of the <br /> service station itself." "It was decided to not go any deeper." "The levels remained in the <br /> 200 to 250 p.p.b.'s of hydrocarbons." "Communications between myself, Chief Baker and <br /> the licensed site professional were not at best." "Chief Baker and I got the impression that <br /> he was just going to remove the tanks and back fill." "I called D.E.P.." "It turns out that <br /> he was allowed to back fill." "What most likely will happen is that Mr. DiMaggio is going <br /> to have to put in gas venting wells around the perimeter of the service station and pump <br /> the air out of the ground, go through carbon filtration and out where it's going to be <br /> monitored." "The M.C.P. is going to have to deliver a response action plan to D.E.P. <br /> which they have to approve." <br /> Acknowledged. <br /> 8.) William's subdivision plan - Mr. McQuaid relayed, "This is a 3 lot definitive <br /> subdivision plan." <br /> Mr. Ball motioned to approve the definitive plan as submitted, Mr. Doherty <br /> seconded, all agreed. <br /> 9.) Fee waiver request, James Lanagan - Mr. McQuaid relayed, "Mr. Lanagan is <br /> requesting the Board incorporate November and December into his 1999 license." <br /> Mr_ Ball motioned to grant a 1999 Installer's License to James Lanagan said <br /> license shall incorporate the months of November and December 1998, Mr. Doherty <br /> seconded, all agreed. <br />
