MINUTES June 10, 1999 2 BOARD OF HEALTH <br /> Stone to hang a sign stating that all produce must be washed prior to <br /> consumption. <br /> Mr. Cram stated his concerns with the conditions at the Flea Market. <br /> I <br /> Mr. Doherty motioned to grant a 1999 retail food permit to Mr. Kucha-Stone for <br /> the sale of produce at Dick & Ellie's Flea Market with the condition that a sign <br /> be posted stating that all produce must be washed prior to consumption. Mr. <br /> Ball seconded the motion. Mr. Cram was opposed. Motion passed. <br /> 5. Copy of letter from Weston & Sampson to Crowell re: landfill closure status <br /> Board acknowledged. <br /> 6. Copy of memo to Selectmen re: SEMASS Contract Amendment <br /> Board acknowledged. <br /> 7. Letter re: water supply-- 134 Ashumet Avenue <br /> Board acknowledged. <br /> 8. Variance request: 67 Great Pines Drive <br /> Applicant is requesting a variance of 2' from the proposed leaching to the <br /> existing well of house #61, a variance of 40'from the proposed leaching to the <br /> existing well of house #11 and a variance of 32'frorn the proposed leaching to <br /> the existing well. <br /> Mr. Cram motioned to grant the variances as requested. Mr. Doherty seconded <br /> the motion. Motion passed. <br /> Variance request: 401 Cotuit Road <br /> Applicant is requesting a variance of 35'from existing well of"Plot #47" and 32' <br /> from the existing well of"Lots #47 and 4E". <br /> Mr. Doherty motioned to deny the requested variances based on unclear <br /> information noted on the submitted plan and letter. Applicant will be required <br /> to submit a corrected plan and cover letter at which time Mr. McQuaid will have <br /> the authority to approve the variance requests. Mr. Cram seconded the motion. <br /> 9. Emergencv Failure Certification: 12 Radcliff Road <br /> Mr. McQuaid stated that the system is overflowing to the ground and <br /> recommends that the reserve be installed and stripped per submitted plan. <br /> Board members forwarded their signatures to the Emergency Failure <br /> Certification form. <br /> Emergency Failure Certification: 8 Radcliff Road <br /> Mr. McQuaid stated that the leach pit is overflowing to the ground and <br /> recommends that a new distribution box and 2 500-gallon drywells be installed. <br /> Board members forwarded their signatures to the Emergency Failure <br /> Certification form. <br />
