VI. Proposal Requirements <br /> Proposal procedure shall be in accordance with the provision of Mass. General <br /> Laws Chapter 7, Section 38A '/2. The following list of items are required for <br /> submittal, in the order listed: <br /> 1. Letter of transmittal, indicating the designer's desire to be considered for the <br /> job, listing sub-consultants to be employed and signed by a principal of the <br /> firm making the proposal. <br /> 2. Summary Statement (Must be less than 10 pages, exclusive of attachments), <br /> detailing the reasons why your firm should be chosen for the job. At <br /> minimum, this statement should include a description of your particular <br /> qualifications for this job. This statement should describe your approach to <br /> this project and explain your skills and experiences. THE STATEMENT <br /> SHOULD DIRECTLY ADDRESS MINIMUM AND COMPARATIVE <br /> EVALUATION CRITERIA LISTED BELOW IN SECTION IX, <br /> EVALUATION OF PROPOSAL. <br /> 3. Description of your firm's size, professional capabilities to undertake the <br /> project in a timely manner, organizational structure and financial stability. <br /> This would include all sub-consultants. <br /> 4. List your three (3) most recent projects of a similar nature. If a principal <br /> was involved with a project of a similar nature while employed at another <br /> firm, identify the project and name of the other firm. In a table format, <br /> disclose the following: <br /> a. Name, location and parry for whom the work was conducted. <br /> b. Current status of the project (completion date). <br /> c. Construction cost. <br /> d. Phone number and contact. <br /> e. Address of reference. <br /> 5. Affirmative hiring goals. <br /> 6. The applicant must complete and submit the form"Application to Designer <br /> Selection Board," which is available at the Office of the Board of <br /> Selectmen. <br /> 7. Ten (10) copies of the proposal shall be submitted in addition to two <br /> originals. <br />
