Parents/guardians of students requiring medication, please review the following regulations: <br /> • Whenever possible, medicinal preparations should be given at home. <br /> • With certain conditions such as epilepsy, diabetes, cystic fibrosis, etc., when medication <br /> must be taken during school hours, the school regulations require a physician's written <br /> order and parent/guardian authorization for school personnel to supervise the taking of <br /> medicine for the condition. <br /> • The medicine must be in its own prescription bottle or its own over-the-counter bottle <br /> with a clear set of written instructions. <br /> • Parents/guardians will provide a month supply of the necessary dosage for one course of <br /> medication. This will be repeated weekly, as necessary, for students on a continuing <br /> medication plan. <br /> • Medications prescribed for three(3) times a day will not be given in school. <br /> • Medications will not be expected to be sent home every day. <br /> • School personnel are not to assume the responsibility for the administration of medicine. <br /> Any medication must be left in the Health Office and never taken in the classroom. <br /> • School personnel are not to assume the responsibility for the administration of medicine <br /> by injection except by special circumstances, e.g., anaphylactic reactions, allergic <br /> reactions. <br /> Parents/guardians are required to notify the School Nurse regarding their child's medical health history <br /> regarding diabetes, allergies, seizures, etc. This allows the school to develop a health care plan if <br /> necessary. Parents/guardians are also asked to contact the classroom teacher in writing of any such <br /> medical issue and what may be required by the teacher(s) in regards to their child. This information will <br /> remain confidential. <br /> Please notify the School Nurse if your child is on a permanent medication at home even if the school does <br /> not have the responsibility of distributing medication during the school day. This may be important <br /> information for the School Nurse to monitor for effectiveness of medication, possible side effects and <br /> adverse reactions. <br /> DOCUMENTATION <br /> A medication order from a licensed prescriber for a long-term prescription medication <br /> must contain the following information: <br /> • The student's name <br /> • The name and signature of the licensed prescriber and business <br /> and emergency phone numbers <br /> • The name, route and dosage of medication <br /> • The frequency and time of medication administration <br /> • The date of the order <br /> • The diagnosis and any other medical condition(s) requiring medication, if not a <br /> violation of confidentiality or if not contrary to the request of a parent/guardian or <br /> student to keep the information confidential <br /> • Specific directions for administration. <br /> 53 <br />
