For short-term prescription medications, i.e., those requiring administration for ten (10) school days <br /> or fewer, the pharmacy labeled container may be used in lieu of a licensed prescriber's order. -" " <br /> The School Nurse shall ensure that there is a written authorization by the parent/guardian which contains: <br /> • The parent's/guardian's printed name and signature and a home and emergency <br /> phone number; <br /> • A list of all medications the student'is currently receiving, if not a violation of <br /> confidentiality or contrary to the request of the parent/guardian or student that <br /> such medication not be documented; <br /> • Approval to have the School Nurse or school personnel designated by the School <br /> Nurse administer the prescription medication; <br /> • Persons to be notified in case of a medication emergency in addition to the <br /> parent/guardian and licensed prescriber. <br /> Following the Board of Registration's Nursing Protocol,there is no distinction or requirements <br /> for distribution between prescription medications and over-the-counter medications (e.g. <br /> aspirin,Tylenol,cough drops,salve,etc.). These must be delivered by the parent/guardian to <br /> the Nurse's Office and not brought in by the child. <br /> TRANSPORTATION OF MEDICATION <br /> STUDENTS ARE NOT ALLOWED TO HAVE IN THEIR POSSESSION ANY <br /> MEDICATIONS. PRESCRIPTION AS WELL AS OVER-THE-COUNTER,WHILE <br /> ON SCHOOL GROUNDS OR ON THE SCHOOL BUS/VAN. <br /> A parent/guardian or parent/guardian-designated responsible adult shall deliver all prescription <br /> medications to be administered by school personnel or to be taken by self-medicating students, if required <br /> by the self-administration agreement(105CMR210.006 (B)), to the School Nurse or other responsible <br /> person designated by the School Nurse. <br /> • The prescription medication must be in a pharmacy or manufacturer labeled container. <br /> • The School Nurse receiving the prescription medication shall document the quantity of the <br /> medication delivered. <br /> • In extreme circumstances, as determined by the School Nurse, the medication may be <br /> delivered by other persons provided they have been designated by the parent/guardian to <br /> do so. <br /> PARENTAL DISPENSING OF MEDICATION <br /> The administration of medications may not be delegated with the exception of epinephrine or other <br /> medication to be administered in a life-threatening situation where the child has a known allergy or pre- <br /> existing medical condition. There must be an order for administration of the medication from a licensed <br /> prescriber and written parental consent. If necessary at other times, parents will be contacted to dispense <br /> medication to their child. <br /> 54 <br />
