The Contractor will submit the draft ('pencil') requisition simultaneously to SMMA and <br /> Chartwell 24 hours in advance of each requisition meeting. <br /> q 9 <br /> 2. Requisition meetings will take place at the site on the Wednesday prior to the end-of- <br /> the-month Building Committee meetings. The latter are normally held on the last <br /> Tuesday of every month. <br /> 3. Draft requisition statements are to be made out in full; that is, the amount sought for <br /> each line item will be stated, together with the completion percentages, as will the <br /> summary page. The cut-off date for each statement will be the 25th day of each <br /> month. <br /> 4. Amounts for labour and materials will be separately identified consistant with the <br /> Schedule of Values (when finalised). If monies are included for off-site materials, the <br /> draft application will be accompanied with supporting documents including a <br /> certificate of insurance, a clear statement of transfer of title and the location of the <br /> materials concerned. Visual inspection will also be necessary, subject to reasonable <br /> limits. <br /> 5. If requisitions include for work not performed or materials not on site as of the date of <br /> the requisition meeting, SMMA will give guidelines to the Clerk of the Works. If the <br /> guidelines are satisfied by the Contractor by 12:00 on the Friday prior to the next <br /> Building Committee meeting, then the sums sought (as may be modified by SMMA's <br /> guidelines) will be included in the certificate for the month in question. If the <br /> guidelines are not satisfied by the deadline, then the line-items concerned will be <br /> trimmed back to SMMA's position as existed on the date of the requisition meeting. <br /> Page 1 of 2 Interim Payment Procedure June 22, 1995 <br />
