Evaluate the extent to which each responsibility/goal has been achieved by placing <br /> the appropriate rating in the space provided. <br /> EVALUATIONEEY: <br /> 4 DISTINGUISHED <br /> Performance is clearly of exceptional quality:overall contribution is noticeably outstanding. <br /> 3 COMMENDABLE. <br /> Performance generally exceeds professional standards and position requirements; overall <br /> contribution is above expectation. <br /> 2 COMPETENT <br /> Pcrlixmancc generally meets professional standards and pcsitiom requirements: improvement is <br /> necessary in one or more areas to consistently sauly ioh requirements. <br /> 1 UNSATISFACTORY <br /> Performance is below the level of acceptability; development/improvement plans should be <br /> instituted immediately. <br /> • COMMENTS: 'Comments"sections have been prnvidcd so that managers can include specific <br /> examples of performance which have contributed to the rating of a particular responsibility or goal. <br /> While managers are encouraged to provide specific fealhack on every performance item, only <br /> those items receiving extremely high or low scores(i.e., 4 - Distinguished or I - Unsatisfactory) <br /> must be accompanied by a brief written explanation. <br />
