Attach copies of <br /> most recent recorded Deed. Tax 8111 or Assessor' s Certification. � <br /> Deed of property recorded in Barnstable County Registry of Deeds, <br /> Book, Page or Land Court Certificate of Title Number, Location 1 <br /> and Description of Property. Assessor' s Map, Block, zoning <br /> District, Amount of Time owned. Section of the zoning Bylaw <br /> which requires the permit you seek, present us of property, <br /> proposed use of property, signature of owner or authorized <br /> representative. Attach written authorization signed by owner. " <br /> After brief discussion, the chairman entertained a motion be <br /> made in this regard. <br /> MOTION: John Kuchinski made a Motion to accept the revised <br /> Town of Mashpee Planning Board Special Permit form; which Motion <br /> was seconded by James Dorgan and so voted unanimously. <br /> i� <br /> Planning Board Members expressed their appreciation for a <br /> job well done to the Planning Department Administrative <br /> Assistant, Myra Suchenicz. (This matter concluded at 8: 00 p.m.. ) <br /> Signature of Taking/Conversion Plans <br /> DPW Director--Greg Taylor - Algonquin Avenue, Pickerel Cove <br /> Road & Circle, Roads in Mashpee Shores Subdivision, Ashers <br /> Heights Subdivision � . <br /> Note: There were approximately six (6) people in <br /> attendance-including Stephen Cashman, 230 Algonquin Avenue <br /> Mashpee, MA; Joseph Burke, 41 Pondview Avenue, Mashpee, MA; and, ' <br /> Robert Robichaud, Shorewood Drive, Mashpee, MA. References were <br /> being made to a Plan entitled., "Taking Plan of Land in Mashpee, <br /> The Chairman noted request had been made by Mr. Taylor for <br /> signatures of both the Board of Selectmen and Planning Board. <br /> { <br /> Mr. Rowleycommented said layout plan has been filed with <br /> y p <br /> the Planning Board, and that the Planning Board should be <br /> prepared to deliver a report to the Town Meeting for acceptance, <br /> and that the Board signature would indicate that said plan had <br /> been filed ro rl <br /> P pe Y- <br /> The Chairman recognized a member of the public, an abutter <br /> g p :. <br /> to Algonquin Avenue, who stated that the plan being proposed by <br /> the Board of Selectmen is not acceptable to the abutters of said <br /> Algonquin Avenue due to the cost. He stated the cost to each <br /> abutter would be Thirty Five Hundred ($3, 500 . 00) Dollars.
