This Department completes first time applications <br /> COMMUNITY HEALTH CENTER $3,694.00 and those that have a recertification that may develop <br /> Primary care services into an emergency situation or needs documents faxed <br /> to Fuel Assistance. Fuel Assistance eligibility is <br /> FALMOUTH SERVICE CENTER $8.020.00 determined by South Shore Community Action in <br /> Food Pantry services and emergency financial Plymouth.
