IISh}IEE <br /> BOARD OF HEALTH <br /> 15 GREAT NECK ROAD NORTH <br /> MIASHFEE,MA 02549 <br /> Abu (508)539-1418 <br /> i <br /> i <br /> I <br /> To All Licensed Food Establishments: <br /> On January 20, 1994, the Mashpee Board of Health amended the Food Service <br /> Establishment requirements, to include the following provisions: <br /> In order to ensure public awareness of the accountability and proper reporting <br /> procedures for sanitation and health related issues concerning Food Service <br /> Establishments, all applicants granted a Food Service Establishment Permit are <br /> required to prominently display "NOTICE TO CONSUMERS" placards, as <br /> specified by the Board of Health or it's agents. Such notice is to be provided to <br /> the permit holder by the Board of Health, in sufficient quantities as outlined <br /> below. <br /> Each permit holder must display the attached "NOTICE TO CONSUMER" <br /> placard(s) so as to be clearly visible to patrons while entering and exiting the <br /> establishment. This applies to all entrances and exits normally utilized by <br /> patrons. Specifically excluded from this requirement are exits only used by <br /> employees, and emergency exits. A "NOTICE TO CONSUMER" placard must <br /> also be clearly displayed at each point of sale within the establishment. For the <br /> purposes of this requirement, a point of sale is defined as a cash register, or <br /> specific location where payment is made for food products or services. <br /> In cases of uncertainty regarding the placement of placards, the Board of <br /> Health or it's agents may specify their quantity and location requirements. <br /> Failure to properly display placards as required, or allowing placards to be <br /> obstructed, may be punished by a fine of not less than $50.00 for the first <br /> offense, $100.00 for the second offense, not less than $150.00 and not to <br /> exceed to $300 00 or permit revocation or suspension for the third and <br /> subsequent offenses. In no case will a permit holder be fined or cited for failure <br /> of this requirement if the Board of Health does not supply the required quantity <br /> of placards. <br /> This requirement will first take effect on March 1, 1994, and thereafter will be <br /> applicable to al# permit holders while their permit is in effect and valid. <br /> i <br /> i <br /> i <br />
