MINUTES February 17, 1994 5 BOARD OF HEALTH 8 . ) Perc extension request - 156 Popponesset Island Road - Mr. McQuaid relayed, "This is a first extension request. " "The perc expired on October 22 , 1993 therefore, the exten- sion will expire in April of 1994. " Mr. Ball motioned to grant the 6 month perc extension from date of expiration as requested, Mr. Greelish seconded, all agreed. FOR DISCUSSION SITE VISIT - 276 MONOMOSCOY ROAD, 2/19/94 @ 10: 00 a.m. Board members agreed to meet at the site on the sched- uled date. CORRESPONDENCE FROM WATER DISTRICT RE: 4 TRI-TOWN CIRCLE Mr. Doherty stated, "We received an answer from the Wa- ter District in our request for dates. " Mr. Ball stated, "I would like to note that a typo- graphical error was made on one of the dates submitted by the Water District. " "Ms. Jean Thomas stated the date which service to the hydrant was made should state October 8, 1993 not October 18, 1993 . " Mr. Doherty commented, "According to all the information we have Commonwealth Electric indicated that on September 24, 1993 the power was shut off. " "Some time prior to October 8, 1993 we were made aware of the situation. " "On October 8 , 1993 connection was made to the hydrant, on October 15, 1993 we sent Mr. Halpern a certified letter instructing him to hookup to water within 48 hours." "Subsequent to his receiv- ing the letter Mr. Halpern contacted this office to meet with the Board. " "The minutes of the October 28, 1993 reflect that I did tell him that we would stay the 48 hour notice so he could make his appeal. " "The Board then voted to order him to hookup by November 1 , 1993 . " "Also, prior to that power was restored on October 18, 1993 and the hydrant was disconnected on the 19th. " "November 1 , 1993 application to the Water District for permanent hookup was received in par- tial compliance with our order, as reflected in the minutes of October 28 , 1993 . " "Of course we know that the meter was not installed until the 13th of January and on the 14th this Board forwarded a certified letter to Mr. Halpern requesting his attendance at a meeting for a complaint hearing. " Mr. Ball questioned whether the tenants moved out before the 13th or after the 13th. Mr. Greelish responded, "After the 13th. "
