following items: <br /> ( a) The Boards's written findings <br /> regarding Subsection 9.373; <br /> (b) The Board' s decision <br /> regarding the number of bonus <br /> units to be allowed; <br /> (c) Any other conditions imposed <br /> by the Board regarding the <br /> development , including any <br /> required road improvements, <br /> bonding etc . ; <br /> (d) A copy of the master site <br /> plan as approved for the <br /> development including all <br /> items required by Subsection <br /> 9.372 (c ) ; <br /> (e) Copies of the approved <br /> convenants ,restrictions and <br /> deeds to be recorded under <br /> Subsection 9. 333; <br /> (f) Any other items the Board <br /> deems necessary to ensure an <br /> adequate legal public record <br /> of its findings and decision . <br /> Upon the granting of a special permit for an OSID , <br /> or any extensi on , modi f i cat i on or renewal thereof , <br /> the Planning Board shall issue to the owner , and <br /> to the applicant if other than the owner , a copy <br /> of its decision certified by the Board and in <br /> conformance with the requirements of Section 11 , <br /> Chapter 40A of the General Laws .
