of the OSID special permit , the Board shall make its decision and findings, including any conditions deemed necessary to ensure that the proposal conforms to the OSID special permit and protects the public health , safety and welfare , within ninety (90) days of the publ i c hearing. 9. 38 APPLICAT.ION AND REVIEW PROCEDURES FOR PARTIAL OSID - Where open space is to be set aside and permanently preserved in anticipation of an OSID in order to establish the number of base and bonus units which would be transferrable to future developed portions of the OSID, the following application and review procedures shall be required: 9. 381 Pre-Application Conferences - Pre-application conferences with the Planning Board, Conservation Commission and Town Planner are required prior to submission of a formal special permit application . 9. 382 Special Permit Application - The applicant shall submit the required application materials or a special permit for a partial OSID to the Planning Board at a regularly scheduled meeting, a copy of which shall forthwith be given to the Town Clerk by the applicant . Required application materials shall include any necessary forms and fees, the items required by Subsections 9. 371 ( a) and (b) , the items required by Subsections 9. 372 (a) , (b) , (d) , ( e) .and ( i ) , a plan of the site to be preserved in a form which would be acceptable for filing as a preliminary subdivision plan , and any -comments received from the Conservation Commission , Town Planner or other Boards or agencies. 9. 383 Within i x tv-f i ve 65) days .af ter the f i 1 i ng of such application with the Board, the Board shall hold a public hearing for- which proper notice has been given . 9. 384 Based on public comment at the hearing , any comments. received from other town boards and agencies, and on the provisions of 'section 9.3, particularly 'subsection 9 .33 and 9.34, the Board shall make a final decision and findings, including the items listed under Subsection 9 .373 ( a) , ( c ) , (d) , ( e) , (f) , (.) (k) and, {: 1 ) , within ninety (90 ) drays of the public hearing . Any special permit approved for a partial OSID shall include the items listed under Subsections 9.374 ( .a) , (b) , (c) , (e) and (f)
