time is requested in writing by the applicant , the Committee shall make written findings and recommendations regarding the application and transmit them forthwith to the Planning Board. (d) Upon expiration of the forty-five (45) day period, or transmission of the Design Review Committee 's findings and recommendations t'o the Planning Board, whichever is sooner (unless an extension of time has been requested in writing) , an application for a special permit maybe filed with the Planning Board along with any requrred forms and fees, the items specified for the Design Review Committee submission above , and any other materials required by the Planning Board along with receipts indicating that copies of these submission materials have been submitted, to the Board of Health , Conservation Commission , Board of Appeals , Town Planner , Superintendant of Streets, Fire Chief and Police Chief .
