c . Evidence of approval by Massachusetts Department of <br /> Environmental Quality Engineering of any industrial <br /> treatment or disposal system or any wastewater system <br /> treatment 15,000 gallons per day capacity, accompanied <br /> by analysis by a Professional Engineer or Sanitary or <br /> Civil Engineering registered in the Commonwealth of <br /> Massachusetts certifying compliance with Section 5.1 .5 <br /> of this by-law. <br /> 6.6 Action. For uses not requiring' a Special Permit under <br /> Section 4.2, the Water Quality Review Committee shall act <br /> within thirty-one ( 31) days received of application, <br /> approving it by issuing a Certificate of Water Quality <br /> Compliance, if the applicant adequately demonstrated <br /> compliance with the requirements of the Water Resource <br /> District, and rejecting the application otherwise. For <br /> uses requiring a Special Permit under Section 4.2, the <br /> Water Quality Review Committee shall make recommendations <br /> to the Board of Appeals within thirty-five ( 35) days of <br /> receipt of the application, as provided in Section 11, <br /> Chapter 40A, Massachusetts General Laws. <br /> 6. 7 Certificate Review. Each three years the Water Quality <br /> Review Committee shall review compliance with the by-law <br /> and the Certificate of Water Quality Compliance, and, if <br /> appropriate, issue a new Certificate of water Quality <br /> Compliance. All certificate holders shall submit the <br /> following not less than thirty ( 30) days prior to the <br /> expiration of the three-year anniversary date of the <br /> issuance of the current Certificate of Water Quality <br /> Compliance: <br /> a. Description of any changes from the originally submitted <br /> materials. <br /> b. Certification that the waste disposal system has been <br /> inspected by a licensed septic system installer or <br /> treatment plant operator within the preceding ninety <br /> ( 90) days and found to be properly maintained and in <br /> proper operating condition. <br /> C. Results from analysis of leachate or wastewaters as may <br /> be required by the Board of Health. <br /> 7. Enforcement <br /> 7.1 Inspection: These provisions shall be enforced by the <br /> Building Inspector. The Building Inspector or Agent of <br /> the Board of Health may enter upon the premises at any <br /> reasonable time to inspect for compliance with the <br /> provisions of the By-law. Evidence of compliance with <br /> approved waste disposal and removal shall be retained by <br /> the holder of the Certificate of Water Quality Compliance. <br /> 7.2 Violations: Written notice of any violation shall be <br /> provided to the holder of the Certificate of Water Quality <br /> Compliance, specifying a time for compliance including <br /> -6- <br />
