the Commonwealth temporarily for the sole purpose <br /> of showing in dog shows or exhibitions. <br /> Section 4 . 14 . 2 <br /> Duties of the Town Clerk <br /> 1 . General <br /> The Town Clerk is responsible for issuing the certi- <br /> ficates and dog tags; for keeping a record of dog <br /> licenses issued and dogs destroyed by the Dog Officer, <br /> accounting for the money collected for licenses and for <br /> making out a monthly report covering all licenses issued <br /> during the preceding month. <br /> 2. Completing a License for Issuance <br /> a) The following information shall be obtained from all <br /> applicants and maintained for two years on records <br /> kept by the Town Clerk for each license issued: <br /> 1 . Date of issue and no other date <br /> 2 . Tag number <br /> 3 . Name of owner or keeper <br /> 4 . Address and phone of owner or keeper <br /> 5. Name of dog <br /> 6 . Breed <br /> 7. Color <br /> 8 . Age <br /> 9. Issuing Clerk <br /> 10 . Expiration date of most recent rabies shot <br /> 11 . Other data - enter not over 10 descriptive words <br /> as requested by the owner which further identify <br /> the dog, such as a tatoo, number, scars, weight, <br /> etc. <br /> b) Spayed female and neutered male licenses <br /> If an owner requests a spayed female or a neutered <br /> male license and the dog is being licensed for the <br /> first time in the town, proof of spaying and neuter- <br /> ing must be shown by the owner . Proof may be : <br /> 1 . A certificate of spaying or neutering signed by <br /> the veterinarian who performed the operation <br /> 2 . A statement signed under the penalties of perjury <br /> by a veterinarian registered and practicing in <br /> Massachusetts describing the dog and stating that <br /> he has examined the dog and that it appears to <br /> have been spayed or neutered and thereby deprived • <br /> of the power of propagation <br /> -59- <br />
