Review of request for variance -- 368 MonomoscoyMain Street Development <br /> Owners hired"Builder A"to build their home.The septic system was installed by"Builder A".Owners made <br /> a change in contractor on the project & "Builder A" has been non-responsive and will not complete the <br /> Certified Installer Form for the new septic system. Main Street Development (replacement builder) is <br /> requesting to take full responsibility for new septic system. Additionally, the new septic system has been <br /> fully inspected&passed by BOH and the engineer of record. <br /> r. Seabury stated for Title 5 to receive the COC you have to have your certified <br /> engineer's As Built and the sign off from the installer saying that they installed it as per <br /> Ian, We have tried reach the installer, Dom.
