Town of Mashpee Conservation Commission <br /> 16 Great Neck Road North <br /> Mashpee, MA 02649 <br /> may require further deliberation and that the matter may not <br /> be ready for a final decision at the next hearing. <br /> It was acknowledged that the applicant is aware of the peer <br /> review findings and that the project remains open for further <br /> discussion and potential revisions based on those concerns. <br /> Vote: Motion passed by unanimous vote. <br /> Steve Cook, Sarah Thornbrugh,Paul Colombo, Sandi Godfrey, Seana Pitt <br /> NOI 25 Elliot Road,William F & Marylou H. Sullivan Trustees, Sullivan Family Trust. 43- <br /> Proposed addition to the second floor of an existing house and upgrade of the septic system. 3400 <br /> (Representative: BSS Design Land Surveying&Planning) (cont. 2.19/3.19) <br /> Revised plans were presented reflecting prior feedback, including a reduction of lawn area to <br /> approximately 703 square feet(under 10% of the lot) and the incorporation of planting beds, crushed stone areas, <br /> and stepping stone walkways. The septic system design was updated to a nitrogen-reducing Singular system, with <br /> revised nitrogen loading calculations remaining within acceptable limits. <br /> Discussion included questions regarding nitrogen concentrations, soil data, and groundwater assumptions. <br /> The applicant explained reliance on nearby soil logs and confirmed adequate separation from groundwater. <br /> Ms. Thornbrugh stated she would not be able to vote due to her absence from prior hearings and lack of a <br /> completed participation form, but she participated in discussion. She raised concerns regarding the extent of <br /> proposed disturbance, the need for multiple regulatory approvals, and a waiver request included in the application <br /> materials. The applicant clarified that the property is not within a floodplain and that certain variances relate to <br /> existing conditions. It was further noted that much of the site disturbance results from required lawn reduction. <br /> The Chair noted that any additional foundation work or excavation beyond what was presented would require <br /> the applicant to return for an amended Order of Conditions. <br /> MOTION: To close an issue at 25 Elliot Road with the following conditions: <br /> 1. Three-year mitigation plan; and <br /> 2. Any future foundation work or excavation would require review by staff and <br /> an amended Order of Conditions. <br /> Motion by: Richard Sahl Seconded by: Sandi Godfrey <br /> Discussion: Mr. Colombo recommended adding a condition requiring <br /> compliance with Sections 3 and 4 of the system's general <br /> certification for general use, including all required reporting <br /> for both the system and the system owner. He noted that this <br /> includes ongoing maintenance, certification, and reporting <br /> obligations, which must be submitted to the Board of Health <br /> and the Commission as the approving authority. <br /> The Commission agreed to incorporate this condition into the <br /> motion, referencing Sections 3 and 4 of the certification letter <br /> dated October 2, 2019. <br /> AMENDED MOTION: The applicant shall comply with all requirements set <br /> forth in Sections 3 and 4 of the Massachusetts Department of Environmental <br /> 4 <br />
