MINUTES April 26, 2001 4 BOARD OF HEALTH <br /> Board members forwarded their signatures to the Emergency Failure <br /> Certification form. <br /> 9. Variance request - 237 Great Pines Drive <br /> The applicant is requesting a variance of 25' from the proposed well to the <br /> proposed soil absorption system, a variance of 14' from the proposed soil <br /> absorption system to the well servicing lot 54 and a variance of 11' from the <br /> proposed well and the existing soil absorption system servicing lot 52. <br /> Mr. Santos motioned to grant the variances as outlined in a cover letter from <br /> Holmes and McGrath, Inc. dated April 20, 2001. Mr. Doherty seconded the <br /> motion. Motion passed. <br /> 10. BOH "order" violation for septic repair - 4 Attaquin Street <br /> Ms. Warden informed Board members of the failed system and subsequent <br /> certified letter to the owners of 4 Attaquin Street ordering them to begin repairs <br /> on their septic system within 14 days of receipt of the letter. To date the <br /> owners have not contacted the Board, nor have they proceeded with repairing <br /> the system. <br /> Mr. Doherty motioned to fine the owners of 4 Attaquin Street $300.00 per day <br /> until the system is corrected due to their failure to respond to the Boards order <br /> to correct. Mr. Santos seconded the motion. Motion passed. <br /> Mr. Doherty further requested that the owners appear before the Board at their <br /> next scheduled meeting. <br /> 11. FYI - Copy of letter to Stendahl Place re: Septic repair "order" <br /> Board acknowledged. <br /> DISCUSSION <br /> a. Draft letter re: E. McQuaid <br /> Board members forwarded their signatures to this correspondence. <br /> b. Health Agent Job Description <br /> Discussion took place regarding the job description for the Health Agent. <br /> C. Orin Evans <br /> Mr. Ball reported that former Assistant Health Agent Orin Evans has indicated <br /> that he would be available to volunteer a few hours a week to help out the <br /> department during the busy weeks ahead. <br /> NEW BUSINESS <br /> a. Emergency Failure Certifications <br /> 9 Nancy Lane <br /> Ms. Warden indicated the leach pit full to the top cover and that the installation <br /> of 2 500-gallon drywells with 4' of stone and new d-box is necessary. <br />
