MINUTES May 31, 2001 6 BOARD OF HEALTH 12. Temporary Food Stand application:.Dino Mitrokostas Mr. Ball motioned to approve a$25.00 Temporary Food Stand permit for Dino Mitrokostas for the three-day Antiques Show to be held at the Boys & Girls Club of Mashpee from June 22 -June 24, 2001. Mr. Santos seconded the motion. Motion passed. 13. Request for I/A unit: 21 Debbie Lane Ms. Warden informed Board members that this was before the Board on May 10, 2001. The Board approved it with variances with the stipulation that the Board receives an operation and maintenance contract for testing requirements for a FAST system. There is going to be a total of 4'of stone, which will be acceptable for the system with a FAST unit. Mr. Ball motioned to grant the installation of a FAST unit for 21 Debbie Lane provided there is going to be a total of W of stone. Mr. Santos seconded the motion. Motion passed. 14. Request to waive fees (Chamber of Commerce: "Taste of Mashpee" & "Night of a Hundred Wines" The Mashpee Chamber of Commerce is requesting that the Board waive the fees associated with their annual "Taste of Mashpee" and "Night of a Hundred Wines". Discussion took place regarding the fact Ms. Warden must perform numerous inspections of the various vendors and the department must cover the cost of the inspection time. Ms. Santos motioned to require a $50.00 temporary food stand permit fee for each event. Mr. Ball seconded the motion. Motion passed. 16. Comm1 Plan Approval with Recirculating Sand Filter: Johnny's Tune & Lube Ms. Warden informed Board members that the plan for Johnny's Tune & Lube had been reviewed by Mr. George Heufelder of the Barnstable County Department of Health and the Environment who felt that the plan was approvable under the Board's regulations. Mr. Ball motioned to approve the commercial site and septic plan with recirculating sand filter for Johnny's Tune and Lube. Mr. Santos seconded the motion. Motion passed. 17. Cape Cod Watershed Team Meeting 6/8/01 at 10:00 Board acknowledged. DISCUSSION a. BOH Administrative Secretary Position and posting Board acknowledged. b. BOH Dept. Accounts Report Board acknowledged.
