Copy of letter to Board of Selectmen re: BOH Administrative Secretary <br /> recommendations <br /> Board acknowledged. <br /> 10. Variance request- 56 Waterway <br /> The applicant is requesting a 10'variance from the proposed soil absorption <br /> system to the front property line. <br /> Mr. Ball motioned to grant the variance as requested in a cover letter from The <br /> Phoenix Group dated June 1, 2001. Mr. Santos seconded the motion. Motion <br /> passed. <br /> 11. Occupancy Permit violation- 14 Oneida Avenue <br /> Ms. Warden gave a brief history of the problems at 14 Oneida Avenue and <br /> stated that Building Inspector Russell Wheeler has notified the owner, Mr. <br /> Kerrigan, that he is in violation of the State Building Code. <br /> Mr. Doherty stated that a licensed septic installer must install the septic system <br /> and that the Board of Health must receive both the engineer and installer's as- <br /> built cards prior to sign-off on the occupancy permit. <br /> Mr. Santos motioned to refuse Mr. Kerrigan's request to waive the Board's <br /> policy to have an as-built card and drawing submitted by a licensed installer <br /> prior to occupancy permit issuance. Mr. Ball seconded the motion. Motion <br /> passed. <br /> 12. Emergency Failure Certifications: <br /> 227 Cotuit Road <br /> Ms. Warden indicated that a failed inspection report was received for the house <br /> at 227 Cotuit Road and, therefore, there is no need for the Board to certify the <br /> system in failure. <br /> Mr. Santos motioned to require engineered plans for the septic system repair at <br /> 227 Cotuit Road due to the proximity of a well to the system. Mr. Ball seconded <br /> the motion. Motion passed. <br /> 14 Webquish Road <br /> Health Consultant Charlotte Stiefel indicated that there is approximately l'of <br /> soil above the leach pit cover, which was saturated with effluent. Due to the <br /> size of the lot and private wells in the area, she is requiring a surveyor to mark <br /> the lot lines and distance to abutting wells. <br /> Board members forwarded their signature to the emergency failure certification <br /> form. <br />
